Eliquis (apixaban) is a commonly prescribed blood thinner used to lower the risk of stroke and blood clots. While it’s effective, many patients find it expensive.
In 2023, Medicare Part D spent about $18.3 billion on Eliquis for roughly 3.9 million people, according to CMS data on the first 10 drugs selected for Medicare price negotiation.
This guide covers everything you need to know about Eliquis cost in 2026, insurance coverage, pharmacy prices, and the availability of generics.
Without insurance, Eliquis costs at least $346 for a 30-day supply of 60 tablets. That is the list price Bristol Myers Squibb (BMS) publishes on its Eliquis pricing page, effective January 2026, and it applies to both the 2.5 mg and 5 mg tablets.
List price is what the drugmaker charges wholesalers. BMS notes that uninsured patients can expect to pay it plus any pharmacy charges, so costs also fluctuate depending on location.
| U.S. price benchmark | 60 tablets (30 days) | 180 tablets (90 days) | Source and date |
|---|---|---|---|
| Eliquis list price | $346 | Not published | BMS, January 2026 |
| Eliquis Direct-to-Patient cash price | $345 | $1,035 | BMS, February 2026 |
| Medicare negotiated price (what Medicare pays, not your copay) | $231 | Not applicable | CMS, effective January 1, 2026 |
Eliquis Starter Pack Price
The Eliquis starter pack holds 74 tablets for the first month of treating a deep vein thrombosis (leg vein clot) or pulmonary embolism (lung clot). Through the BMS Eliquis Direct-to-Patient program, a 74-tablet, 30-day supply costs $426 as of February 2026. New users can get up to 74 tablets free with the 30-day trial card below.
Eliquis 5 mg Price at Walmart, CVS, and Costco (2026)
The Eliquis 5 mg price at Walmart starts at $346 for 60 tablets, or about $5.77 a tablet, without insurance. That is the manufacturer list price Bristol Myers Squibb published in January 2026, and the 2.5 mg tablet costs the same. BMS says uninsured patients can expect to pay the list price plus any pharmacy charges, so the counter price at Walmart, CVS, or Costco can run higher and varies by store.
| Where you buy | 60 tablets (30 days) | Per tablet | Source and date |
|---|---|---|---|
| Walmart, CVS, or Costco (cash, no insurance) | $346 or more | $5.77 or more | BMS list price, January 2026 |
| BMS Direct-to-Patient program | $345 | $5.75 | BMS, February 2026 |
| Pharma Giant (Eliquis 5 mg, Canada) | $157 | $2.62 | Pharma Giant, October 7, 2026 |
Prices in this post were accurate as of October 7, 2026 and may change in the future. Check the product page for the current price.
Compared with the $346 list price, the $157 Pharma Giant price is at least $189 less for 60 tablets, about 55%. For an exact Walmart quote, call the pharmacy. If you have insurance, ask for your copay instead, which is usually much lower.
Cost of Generic Eliquis at Walmart
Generic Eliquis has no price at Walmart because no U.S. pharmacy can sell it until at least April 1, 2028. A shopper searching Walmart for generic apixaban today will find only brand-name Eliquis at $346 or more for 60 tablets.
Generic apixaban has been sold in Canada since June 2022, and Pharma Giant carries both brand-name Eliquis and generic apixaban, with a valid prescription.
Safety Note
Do not skip doses or split your supply to make a bottle last longer because of cost. The Eliquis prescribing information warns that stopping Eliquis early raises your risk of stroke and blood clots. If cost is a problem, talk to your prescriber before changing anything.

How Much Does Eliquis Cost With Insurance?
With insurance, most people pay well under $60 a month for Eliquis. The cost of Eliquis can vary depending on your insurance plan’s drug formulary (the list of drugs your plan covers) and coverage tier.
| Coverage type | Average cost per month | Half of patients pay |
|---|---|---|
| Commercial insurance | $33 to $37 | $10 to $20 or less |
| Medicare | $45 to $51 | $15 to $30 or less |
| Medicaid | $2 to $3 | $0 |
| Medicare Extra Help | $0 to $12.65 | Not reported |

Private Insurance (Employer or Commercial Plans)
Since Eliquis is commonly prescribed for conditions like atrial fibrillation (AFib, an irregular heartbeat) and blood clots (DVT or PE), it’s typically covered. Eliquis is often listed as a preferred brand-name drug, typically in Tier 2 or 3, depending on your plan.
According to Bristol Myers Squibb, people with commercial coverage pay an average of $33 to $37 per month, and half pay $20 or less. If you have commercial insurance, the Eliquis Co-pay Card can bring that down to as little as $10 a month (details below).
But, you might need prior authorization (your plan’s approval before it pays) or have to try another medication first (step therapy). In some cases, your doctor will need to confirm that Eliquis is medically necessary or that more affordable alternatives, like warfarin, aren’t suitable for you.
How Much Does Eliquis Cost With Medicare in 2026?
Medicare patients pay an average of $45 to $51 a month for Eliquis, according to BMS, and the most anyone with Part D pays out of pocket for covered drugs in 2026 is $2,100. Eliquis is typically included in Medicare Part D (prescription drug plans) and Medicare Advantage plans that offer drug coverage. What you’ll pay depends on which stage of your Medicare plan you’re currently in.
Starting January 1, 2026, Medicare pays a negotiated price of $231 for a 30-day supply of Eliquis, down from a 2023 list price of $521, according to CMS. This is what Medicare pays, not your copay. For help choosing a plan, see our guide to the best Medicare Part D plan for Eliquis.
-
Deductible Stage
At the start of your coverage year, if your plan includes a deductible, you’ll need to pay the full cost of Eliquis out of pocket until that deductible is met.
In 2026, the maximum deductible for any Medicare drug plan is $615, according to CMS’s 2026 Part D program instructions. Because your plan’s price for Eliquis is based on Medicare’s $231 negotiated price, it may take about three monthly fills to meet a $615 deductible.
If your Medicare drug plan doesn’t have a deductible, you skip the deductible stage entirely and move straight into the initial coverage phase at the start of the year.
-
Initial Coverage Stage
If your plan has no deductible, or once you’ve met it, you enter the initial coverage phase. At this point, you no longer pay the full price of Eliquis. Instead, you pay only a portion of it, and your plan covers the rest.
In 2026, you typically pay 25% of the drug’s price while your plan pays 75%. Using the $231 negotiated price as an example, 25% works out to about $58 per 30-day fill. Many plans charge a flat copay instead, so check your plan’s drug list for your exact amount.
Only what you pay yourself counts toward the $2,100 cap. If Eliquis is your only drug, you may never hit it: the example above comes to roughly $1,150 a year.
-
Catastrophic Coverage Stage
Once you reach the $2,100 out-of-pocket limit, you enter the catastrophic phase. From this point on, Medicare will cover 100% of your Eliquis costs for the rest of the calendar year.
To summarize Eliquis’ cost with Medicare in 2026:
| Coverage phase | What you pay for Eliquis |
|---|---|
| Deductible phase | Up to 100% of your plan's price (max $615) |
| Initial coverage phase | Around 25% of Eliquis's cost, or your plan's flat copay |
| Catastrophic coverage | $0 (fully covered) after $2,100 out of pocket |
Extra Help/Low-Income Subsidy
If you’re on Medicare and qualify for Extra Help (also known as the Low-Income Subsidy or LIS), your monthly cost for Eliquis may be significantly reduced. The LIS program was expanded in 2024 to include more Medicare beneficiaries.
In 2026, Extra Help copays for brand-name drugs like Eliquis are capped at $12.65, and some people pay $0, depending on their income level and eligibility category. According to Bristol Myers Squibb, people with Extra Help pay between $0 and $12.65 each month for Eliquis. You can apply through the Social Security Administration’s Extra Help page.
Medicaid Coverage
Eliquis is covered by most state Medicaid programs, particularly for individuals being treated for conditions such as atrial fibrillation, deep vein thrombosis (DVT), or pulmonary embolism (PE).
However, coverage terms can vary by state. Some Medicaid plans may require prior authorization or step therapy.
By federal law, Medicaid imposes only minimal out-of-pocket costs for prescription drugs. On average, Medicaid enrollees pay $2 to $3 per month for Eliquis, and half pay nothing, according to BMS. In December 2025, BMS also agreed with the U.S. government to supply Eliquis to Medicaid for free starting January 1, 2026. Your exact copay still depends on your state’s rules.
Contact your state’s Medicaid office directly to understand your exact coverage and any requirements.

1. Eliquis Copay Card
BMS offers an Eliquis Co-pay Savings Card for patients with private insurance. Eligible users can pay as little as $10 for a 30-day supply of Eliquis.
The card covers all but $10 of the copay, up to a set limit. For example, if a patient’s copay is $50, the card covers $40, leaving just $10 for the patient.
Patients can also pay $10 for their first 90-day supply, then as little as $30 for later 90-day refills. The card lasts up to 24 months from activation, with a maximum benefit of $2,000 per year, according to BMS’s terms. You must activate and first use the card by December 31, 2026.
Who Can Use This Offer?
There’s no income requirement to qualify for the Eliquis Co-pay Savings Card. Even higher-income individuals can use it, as long as they have private insurance. However, several important restrictions apply.
You cannot use the copay card if you’re enrolled in any government-funded health program, including Medicare Part D, Medicaid, Medigap, VA, DOD, or TriCare.
These restrictions exist due to the federal Anti-Kickback Statute, which bars drugmakers from paying copays for people on Medicare, Medicaid, or similar programs.
2. Free 30-Day Trial Offer
If you’re new to Eliquis, you can get a free 30-day supply through the manufacturer’s trial program. This one-time Free Trial Card covers up to 74 tablets, enough for your first month, even if your starting dose is higher than usual. You can usually get the card from your doctor or directly from the Eliquis website.
Who Can Use This Offer?
To qualify:
- You must be a first-time Eliquis user with a valid prescription.
- You can’t have filled a paid Eliquis prescription before.
- Your prescriber must plan more than 35 days of treatment.
- You can’t combine this offer with other discounts.
The good news is that there are no insurance restrictions. Even if you’re on Medicare or don’t have insurance at all, you’re still eligible. The card must be used by December 31, 2026.
3. Eliquis Direct-to-Patient Program
If you’re uninsured or your plan doesn’t cover Eliquis, you can buy it through BMS’s Eliquis 360 Support program for $345 per 60 tablets or $1,035 per 180 tablets (BMS, February 2026), shipped to your home. It is not open to people with Medicare drug coverage, and payments don’t count toward any deductible.
4. Eliquis Patient Assistance Program
Bristol Myers Squibb’s Patient Assistance Foundation (BMSPAF) offers free Eliquis to eligible individuals.
Who Can Use This Offer?
To qualify for free Eliquis through the BMS Patient Assistance Foundation, you typically must:
- Be a US resident
- Have no prescription insurance or insurance that doesn’t cover Eliquis, or have Medicare and have already spent at least 3% of your yearly household income on prescriptions this year
- Have household income below the foundation’s limits, which change yearly (confirm at org)
- If you’re 65 or older with income below 150% of the federal poverty level, apply for Extra Help first
- Provide documentation, such as a completed application form and a proof of income (e.g., recent tax return or pay stubs)
If you’re approved, the program ships your Eliquis, either to your home or your doctor’s office, completely free. Most approvals last for a set period, like one year, and you’ll need to reapply after that.
Is There a Generic for Eliquis?
Yes, the generic name for Eliquis is apixaban, but generic apixaban is not sold in U.S. pharmacies yet. The FDA approved the first generic versions of Eliquis in December 2019, but patent rulings mean generic makers cannot launch in the U.S. until at least April 1, 2028, according to a Bristol Myers Squibb and Pfizer statement on the Eliquis patent ruling.
| Generic apixaban | United States | Canada |
|---|---|---|
| Approved? | Yes (FDA, December 2019) | Yes |
| Sold in pharmacies? | Not before April 1, 2028 | Yes, since June 2022 |
| Can U.S. patients get it? | Not from U.S. pharmacies | With a valid prescription, through a licensed Canadian pharmacy |

The Bottom Line
Eliquis has a list price of $346 for a 30-day supply in 2026, but prices vary by pharmacy. With private insurance, most people pay an average of $33 to $37, or as little as $10 with the co-pay card. Extra Help and Medicaid can reduce the cost to just a few dollars or nothing at all.
Buying from Canadian pharmacy platforms like Pharma Giant can lower the price to around $157 for 60 tablets (as of October 7, 2026). You can also use manufacturer savings programs, including copay cards, a free 30-day trial, direct-to-patient pricing, and a patient assistance program for eligible low-income individuals.
Whichever route you choose, get Eliquis from a licensed pharmacy, take it exactly as prescribed, and talk to your prescriber before any change. Check current prices on Pharma Giant.
