What is Avandia?
Avandia (rosiglitazone maleate) is a medication used to help control blood sugar levels in adults with type 2 diabetes, a condition where your body doesn’t use insulin properly.
It’s important to use Avandia along with a healthy diet and regular exercise to see the best results.
Avandia works by making your body more sensitive to the insulin it already produces, but it doesn’t increase the amount of insulin your body makes.
This medicine is not suitable for people with type 1 diabetes, a condition where the body doesn’t produce insulin, or for treating diabetic ketoacidosis, a serious diabetes complication.
People with certain heart conditions should not use Avandia.
How Does Avandia Work?
Avandia works by helping your body use insulin more effectively. Insulin is a hormone that allows sugar (glucose) to enter your cells and be used for energy.
In people with type 2 diabetes, the body doesn’t respond well to insulin, leading to high blood sugar levels.
Avandia helps improve the body’s sensitivity to insulin by activating a specific protein in cells, known as PPAR-gamma.
This protein is found in areas of the body like fat tissue, muscles, and the liver.
When activated, it helps regulate genes that control how the body processes glucose and fats, ultimately lowering blood sugar levels.
Avandia does not cause low blood sugar (hypoglycemia) on its own.
Dosage Information and Usage
Dosage Form and Strengths of Avandia
- 2 mg: Pink, pentagonal, film-coated tablet with “SB” on one side and “2” on the other.
- 4 mg: Orange, pentagonal, film-coated tablet with “SB” on one side and “4” on the other.
- 8 mg: Red-brown, pentagonal, film-coated tablet with “SB” on one side and “8” on the other.
Usage
Avandia should be taken exactly as your doctor prescribes.
The usual starting dose is 4 mg daily, which can be taken either as a single dose or divided into two doses.
Your doctor might adjust the dose depending on how well your blood sugar is controlled. Avandia can be taken with or without food.
It typically takes about two weeks for Avandia to start lowering your blood sugar, with the full effect usually seen within two to three months.
It’s important to take Avandia regularly, follow your diet and exercise plan, and monitor your blood sugar as directed by your doctor.
If you miss a dose, take it as soon as you remember, but do not double up on doses.
Storage Information
Store Avandia at room temperature, around 25°C (77°F).
It’s okay if the temperature occasionally varies between 15°C and 30°C (59°F to 86°F).
Make sure to keep the medication in a tight, light-resistant container to protect it from light and moisture.

Warnings and Precautions
- Cardiac Failure: Avandia can cause fluid retention, leading to or worsening heart failure. Patients should be monitored for symptoms like swelling, shortness of breath, or fatigue. If these symptoms appear, treatment for heart failure should be started, and the dosage of Avandia may need to be reduced or stopped.
- Risk in Heart Failure Patients: Avandia increases the risk of cardiovascular events in patients with congestive heart failure (CHF) classified as NYHA Class I or II. In a study, patients with diabetes and mild CHF had a higher incidence of cardiovascular problems, including heart failure, when treated with Avandia. Starting Avandia in patients with severe heart failure (NYHA Class III or IV) is not recommended, and it’s important to consider stopping the medication in patients experiencing a heart attack.
- Myocardial Ischemia: Avandia may increase the risk of myocardial ischemia, a condition where the heart muscle receives less oxygen due to reduced blood flow. This can lead to chest pain or heart attacks. A study showed a higher risk of such events when Avandia was used, especially in combination with insulin or nitrate therapy. It’s important to monitor patients closely for symptoms like chest pain or shortness of breath.
- Use with Insulin: Combining Avandia with insulin increases the risk of heart failure and myocardial ischemia. In several studies, patients taking both Avandia and insulin had higher rates of heart-related problems. This combination is not recommended, and alternative treatments should be considered for patients needing insulin therapy.
Overdosage
If someone takes too much Avandia, there isn’t a lot of information on what might happen.
In studies where people were given a single dose as high as 20 mg, it was generally well-tolerated without serious issues.
However, if an overdose does occur, the person should receive supportive care based on their specific symptoms and condition at the time.
Drug Interactions With Other Medications
Avandia can interact with other medications, which may affect how well it works.
One type of interaction involves drugs that affect a liver enzyme called CYP2C8.
If a medication that inhibits (slows down) CYP2C8, like gemfibrozil, is used with Avandia, it can increase the level of Avandia in the blood.
This could make the effects of Avandia stronger.
On the other hand, a medication that induces (speeds up) CYP2C8, like rifampin, can lower the level of Avandia in the blood, reducing its effectiveness.
If you start or stop taking any of these drugs while on Avandia, your healthcare provider may need to adjust your diabetes treatment based on how well your blood sugar is controlled.
