What is Avapro?

Avapro (irbesartan) is a medication used to treat high blood pressure, which is also called hypertension.

Lowering high blood pressure helps reduce the risk of serious problems like strokes and heart attacks.

Avapro works by relaxing blood vessels so that blood can flow more easily. It can also be used to treat a condition called diabetic nephropathy, which is kidney damage caused by type 2 diabetes.

This helps slow down the progression of kidney disease in these patients. Avapro can be taken independently or with other medications that lower blood pressure.

How Does Avapro Work?

Avapro works by blocking a substance in your body called angiotensin II.

Angiotensin II makes your blood vessels tighten, which raises blood pressure.

Avapro prevents this from happening by blocking the angiotensin II receptor, so your blood vessels stay relaxed and your blood pressure goes down.

This also helps reduce the strain on your heart and kidneys, especially if you have conditions like hypertension or diabetic kidney disease.

Unlike some other blood pressure medications, Avapro doesn’t affect other important body processes, so it’s specifically targeted at lowering blood pressure.

Dosage Information and Usage

Dosage Form and Strengths of Avapro

  • Avapro 75 mg: White to off-white, oval-shaped, film-coated tablet with a heart on one side and “2871” on the other.
  • Avapro 150 mg: White to off-white, oval-shaped, film-coated tablet with a heart on one side and “2872” on the other.
  • Avapro 300 mg: White to off-white, oval-shaped, film-coated tablet with a heart on one side and “2873” on the other.

Usage

Avapro is usually taken once a day, with or without food.

The starting dose for most people is 150 mg daily, which can be increased to 300 mg daily if needed to control blood pressure.

For patients with kidney problems due to type 2 diabetes, the typical dose is 300 mg once a day.

In some cases, like when the body has lost a lot of fluids or salt, a lower starting dose of 75 mg may be recommended.

Avapro can be used alone or combined with other blood pressure medications to help manage hypertension effectively.

Side Effects

Common Side Effects

  • High potassium levels
  • Dizziness, especially when standing up
  • Decreased blood pressure

Serious but Rare Side Effects

  • Extremely low blood pressure (hypotension), which can cause fainting or dizziness
  • Kidney damage, which might show as feeling weak, tired, or confused

Warnings and Precautions

  • Fetal Toxicity: Avapro can harm an unborn baby if taken during pregnancy, especially in the later stages. It may cause serious issues like poor kidney function, lung problems, and skeletal deformations in the fetus. If you become pregnant while taking Avapro, stop the medication immediately and inform your healthcare provider.
  • Hypotension in Volume or Salt-Depleted Patients: Avapro can cause very low blood pressure in people who have low levels of fluids or salt in their bodies, such as those on high doses of diuretics. To avoid this, make sure your fluid and salt levels are balanced before starting Avapro, or your doctor may start you on a lower dose.
  • Impaired Renal Function: Avapro can cause changes in kidney function, including acute kidney failure, especially in people with certain conditions like kidney disease or severe heart failure. Your doctor will monitor your kidney function regularly while you are taking Avapro, and they may adjust or stop your treatment if any serious issues arise.

Overdosage

If someone takes too much Avapro, it is expected to cause very low blood pressure (hypotension) and a fast heartbeat (tachycardia), though a slow heartbeat (bradycardia) might also happen.

High doses, even up to 900 mg daily for 8 weeks, have been tolerated in some cases.

However, in case of an overdose, it’s important to seek medical attention immediately. Avapro cannot be removed from the body through dialysis.

Drug Interactions With Other Medications

  • Agents Increasing Serum Potassium: Taking Avapro with other medications that raise potassium levels can lead to high potassium in the blood, which can be dangerous. It’s important to monitor potassium levels if you’re using such medications together with Avapro.
  • Lithium: Using Avapro with lithium may cause an increase in lithium levels, leading to lithium toxicity. If you are taking both medications, your healthcare provider should regularly check your lithium levels to avoid potential toxicity.
  • Nonsteroidal Anti-inflammatory Drugs (NSAIDs) Including Selective Cyclooxygenase-2 Inhibitors (COX-2 Inhibitors): Combining Avapro with NSAIDs, especially in older adults or those with kidney issues, can worsen kidney function or even lead to acute kidney failure. Regular monitoring of kidney function is necessary if these drugs are taken together, as the effects are usually reversible.
  • Dual Blockade of the Renin-Angiotensin System (RAS): Using Avapro with other medications that block the RAS, such as ACE inhibitors or aliskiren, increases the risk of low blood pressure, high potassium, and kidney problems. This combination often doesn’t provide extra benefits, so it’s usually better to avoid it unless specifically advised by your doctor, with close monitoring of blood pressure, kidney function, and electrolytes.

Frequently Asked Questions

Avapro is used to treat high blood pressure (hypertension) and to protect the kidneys in patients with type 2 diabetes who have high blood pressure.
Avapro is not safe during pregnancy. It can cause serious problems for the baby, such as skull abnormalities, kidney failure, and even death. If you become pregnant, you should stop taking Avapro immediately.
Avapro may interact with certain medications, especially if you have diabetes. Before taking Avapro, inform your healthcare provider about all prescription and over-the-counter medicines, vitamins, minerals, herbal products, and supplements you are using.
Common side effects of Avapro include high potassium levels, dizziness, and decreased blood pressure. Serious but rare side effects can include severe drops in blood pressure, kidney damage, and allergic reactions.
Avapro works by blocking the action of angiotensin II, a hormone that causes blood vessels to constrict. This helps to relax and widen the blood vessels, which lowers blood pressure and reduces the strain on the heart.

Drinking alcohol while taking Avapro can enhance its blood pressure-lowering effects, which may lead to headaches, dizziness, lightheadedness, fainting, or changes in heart rate. It’s best to limit alcohol consumption while on Avapro.
If you miss a dose of Avapro, take it as soon as you remember. If it’s almost time for your next dose, skip the missed dose and continue with your regular schedule. Do not double up on doses to make up for a missed one.
Signs of a serious allergic reaction to Avapro include difficulty breathing, wheezing, a racing heart, fever, swollen lymph nodes, and swelling of the face, lips, mouth, tongue, or throat. If you experience any of these symptoms, stop using Avapro and seek medical help immediately.

Store Avapro at room temperature, between 68°F to 77°F (20°C to 25°C). It can be exposed to temperatures between 59°F to 86°F (15°C to 30°C) but avoid extreme temperatures.
Yes, Avapro can sometimes cause your blood pressure to drop too much, leading to low blood pressure.
While taking Avapro, you should avoid using diuretics (water pills), digoxin, or blood thinners without consulting your doctor. Inform your healthcare provider about all the medications you are using to prevent potential interactions.
Yes, Avapro can be taken if you have diabetes, especially if you have hypertension or diabetic nephropathy. It helps manage blood pressure effectively in patients with type 2 diabetes.
You will need to take Avapro as long as your doctor prescribes it. Typically, it is taken once a day, and your doctor may recommend taking your first dose before bedtime to minimize dizziness.
Yes, Avapro can interact with NSAIDs (nonsteroidal anti-inflammatory drugs). This combination may affect kidney function and reduce the effectiveness of Avapro in lowering blood pressure.
Yes, Avapro can cause dizziness. This is more likely to occur when you first start taking the medication or if your blood pressure drops too low.
If you overdose on Avapro, you may experience low blood pressure (hypotension) and a rapid heart rate (tachycardia). In severe cases, bradycardia (slow heart rate) could occur.
Avapro can affect kidney function, especially when used with NSAIDs or in patients who are elderly, fluid-depleted, or have existing kidney issues. This can lead to reduced kidney function or kidney failure, but these effects are usually reversible.
Avapro can increase blood levels of lithium, potentially leading to lithium toxicity, which can be severe and life-threatening.
Avapro is generally safe for elderly patients. The medication’s elimination half-life is not significantly different in older adults compared to younger individuals, though plasma concentrations may be somewhat higher in the elderly.
