What is Caverject?
Caverject (alprostadil) is a prescription medicine used to treat erectile dysfunction (ED), which is the inability to get or keep an erection firm enough for sexual activity.
It contains alprostadil, a natural substance that helps improve blood flow to the penis. This medication is injected directly into the penis to help achieve an erection.
Caverject is also used in some medical tests to diagnose the cause of ED. It is not intended for use by women or children.
Before using Caverject, it’s important to inform your healthcare provider about your medical history and any other medicines you’re taking to avoid potential interactions.
How Does Caverject Work?
Caverject works by using alprostadil, a substance that helps relax the blood vessels in the penis.
When injected, alprostadil causes the smooth muscles in the penis to relax and the blood vessels to widen.
This allows more blood to flow into the penis, leading to an erection. It also helps trap the blood by compressing the veins, which maintains the erection.
Caverject makes it easier for the blood to fill and stay in the penis, which helps achieve and maintain an erection.
Dosage Information and Usage
Dosage Form and Strengths of Caverject
- Available in single-dose vials.
- Strengths: 20 mcg/vial and 40 mcg/vial.
Usage
Initial Administration
- Your first dose of Caverject should be administered in your healthcare provider’s office. This allows for monitoring and instruction on the proper use of the medication.
- Your doctor will demonstrate the injection technique, including how to prepare the medication and administer the injection. Pay close attention and ask any questions to ensure you understand the process.
Preparation
- Collect all necessary supplies, including Caverject vial, syringe, and needle. Ensure your hands and the surface where you are working are clean.
- Check that the Caverject vial has not expired and the solution is clear. Do not use if it is discolored or contains particles.
- Draw Medication:
- Clean the top of the Caverject vial with an alcohol swab.
- Attach the needle to the syringe and draw the prescribed amount of Caverject into the syringe.
- Remove any air bubbles by tapping the syringe and pushing the plunger slightly.
Injection Site
- Choose an injection site on the side of the penis, preferably where there are fewer veins or prominent structures. Rotate sites with each use to prevent tissue damage.
- Use an alcohol swab to clean the area where you will inject. Allow the area to dry before injecting.
Administering the Injection
- Insert the needle at a 90-degree angle into the cleaned area of the penis.
- Slowly push the plunger to inject the medication. Avoid injecting too quickly as this can cause discomfort.
- Once the medication is fully injected, withdraw the needle gently and apply slight pressure with a clean cotton swab or gauze to the injection site to minimize bleeding.
Post-Injection
- Dispose of the used needle and syringe in a puncture-resistant sharps container immediately. Do not throw them in regular trash.
- Monitor the injection site for any adverse reactions such as prolonged pain, swelling, or redness.
Frequency of Use
- Use Caverject no more than 3 times per week. Ensure there is at least a 24-hour gap between injections.
- Keep a log of your injections to help manage the frequency and avoid overuse.
Storage Information
Store Caverject vials in a cool, dry place. For 20 mcg vials, keep them between 68°F and 77°F (20°C to 25°C).
Use the reconstituted solution within 24 hours, and do not refrigerate or freeze it. For 40 mcg vials, store them between 36°F and 46°F (2°C to 8°C) until ready to use.
After dispensing, these vials can be stored at or below 77°F (25°C) for up to 3 months or until the expiration date.
Use the reconstituted solution within 24 hours and do not refrigerate or freeze it.
Always use bacteriostatic water for injection to reconstitute Caverject.
Side Effects
Common Side Effects
- Penile pain
Serious but Rare Side Effects
- Priapism (erection lasting over 4 hours)
- Penile Fibrosis (change in penis shape)
- Low blood pressure
- Injection site bleeding
- Heart problems (symptoms like chest pain, dizziness, or nausea)
Warnings and Precautions
- Prolonged Erection and Priapism: An erection lasting longer than 4 hours requires immediate medical help. Prolonged erections or priapism (lasting more than 6 hours) can cause permanent damage if not treated right away. To reduce the risk, use the lowest effective dose and avoid Caverject if you have conditions that increase the risk of priapism, like sickle cell anemia or leukemia.
- Penile Fibrosis: Periodic checks of the penis are needed to spot any signs of fibrosis, a condition where the tissue hardens. If you develop penile angulation or fibrosis, stop using Caverject. The incidence of penile fibrosis in clinical studies was about 3%, and up to 7.8% in long-term studies.
- Hypotension: Using Caverject can lead to low blood pressure. Avoid using it if you have cavernosal venous leakage. Monitor for signs of hypotension and consult your doctor if you have concerns.
- Injection Site Bleeding When Used with Anticoagulants: If you’re on blood thinners like warfarin or heparin, you may experience increased bleeding at the injection site. Apply pressure with an alcohol swab or sterile gauze for 5 minutes to minimize bleeding.
- Cardiovascular Risk Related to Underlying Medical Conditions: Sexual activity can pose cardiac risks for those with heart conditions. Only use Caverject if your heart is healthy enough for sexual activity. A full medical assessment should determine if it’s safe for you.
- Risks of Use in Combination with Other Vasoactive Medications: Combining Caverject with other vasoactive drugs may increase the risk of prolonged erection, priapism, and low blood pressure. The safety of such combinations has not been well-studied.
- Needle Breakage: Use separate needles for reconstitution and injection. Superfine needles used for Caverject can break and may require medical intervention if a piece is left in the penis. Follow proper handling and injection techniques to avoid breakage.
- Risk of Serious Adverse Reactions in Infants due to Benzyl Alcohol Preservative: Caverject contains benzyl alcohol, which can cause serious reactions, including “gasping syndrome,” in infants. This drug is not intended for children.
- Counsel Patients about Sexually Transmitted Diseases: Caverject does not protect against sexually transmitted infections, including HIV. Use appropriate protective measures to prevent transmission of diseases.
Overdosage of Caverject
Overdosage of Caverject was not seen in clinical trials. If too much of the drug is injected into the penis, you should seek medical help immediately.
Doctors will monitor you until the effects, such as prolonged erection or low blood pressure, are resolved. It is important to address any symptoms or side effects promptly with appropriate treatment.
Drug Interactions With Other Medications
The potential for interactions between alprostadil and other medications has not been extensively studied.
