What is Climara?

Climara (estradiol) is a prescription patch that you wear on your skin. It contains estradiol, which is a type of estrogen hormone.

Estrogen is a hormone naturally produced by a woman’s ovaries, but after menopause—when periods stop—the body produces much less of it.

Climara is used to help with symptoms of menopause, like hot flashes, which are sudden feelings of heat, and vaginal changes, like dryness.

It can also be used to prevent weak bones (osteoporosis) after menopause or to treat women who don’t produce enough estrogen before menopause.

How Does Climara Work?

Climara works by providing your body with estradiol, a form of estrogen hormone that is important for maintaining the female reproductive system and characteristics.

Normally, estrogen is produced by the ovaries, but after menopause, the body produces much less of it.

Climara delivers estradiol through the skin, helping to balance hormone levels.

This helps reduce symptoms like hot flashes and vaginal dryness by interacting with specific receptors in your body that respond to estrogen.

Dosage Information and Usage

Dosage Form and Strengths of Climara

  • 0.025 mg per day—each 6.5 cm² system contains 2 mg of estradiol
  • 0.0375 mg per day—each 9.375 cm² system contains 2.85 mg of estradiol
  • 0.05 mg per day—each 12.5 cm² system contains 3.8 mg of estradiol
  • 0.060 mg per day—each 15 cm² system contains 4.55 mg of estradiol
  • 0.075 mg per day—each 18.75 cm² system contains 5.7 mg of estradiol
  • 0.1 mg per day—each 25.0 cm² system contains 7.6 mg of estradiol

Usage

Climara is used to treat symptoms of menopause, such as hot flashes and vaginal dryness, and to prevent osteoporosis (weak bones) after menopause.

The patch should be applied once a week to a clean, dry area on the lower abdomen or upper buttock, avoiding the breast area.

The dosage should be adjusted based on your response, and it’s important to use the lowest effective dose for the shortest time necessary.

Your doctor will regularly check to see if you still need to use Climara. Make sure to rotate the application site each time to prevent skin irritation.

If the patch falls off, reapply it or use a new one to continue the treatment.

Storage Information

Store Climara patches at room temperature, between 66°F and 77°F (20°C to 25°C). It’s okay if the temperature varies slightly, but don’t store them above 86°F (30°C).

Keep the patches in their protective pouch until you are ready to use them.

After using a patch, fold it in half with the sticky sides together and put it in a child-proof container before throwing it away.

Do not flush used patches down the toilet.

Warnings and Precautions

  • Cardiovascular Risks: Using estrogen therapy alone or with progestin can increase the risk of stroke, deep vein thrombosis (DVT), pulmonary embolism (PE), and heart attack (MI). Stop the therapy immediately if any of these conditions occur or are suspected.
  • Stroke Risk: Estrogen-alone therapy may raise the risk of stroke, especially in women aged 50 to 79 years. Stop the therapy if a stroke happens or is suspected.
  • Coronary Heart Disease (CHD) Risk: Estrogen therapy alone does not significantly affect CHD risk, but combined with progestin, it may slightly increase the risk of heart-related problems. Monitor heart health closely.
  • Venous Thromboembolism (VTE) Risk: Estrogen therapy can increase the risk of blood clots in veins (VTE), including DVT and PE. Stop the therapy if these conditions are suspected or confirmed.
  • Endometrial Cancer: Estrogen therapy without progestin can increase the risk of endometrial cancer in women with a uterus. Progestin may reduce this risk. Women should be monitored for abnormal bleeding.
  • Breast Cancer Risk: Combined estrogen and progestin therapy may increase the risk of breast cancer. Regular breast exams and mammograms are essential for monitoring.
  • Ovarian Cancer Risk: Estrogen plus progestin therapy may slightly increase the risk of ovarian cancer. The exact risk level is still unclear.
  • Dementia Risk: Women aged 65 and older using estrogen alone or with progestin may have an increased risk of developing dementia. This risk is higher with combined therapy.
  • Gallbladder Disease: Estrogen therapy can raise the risk of gallbladder disease, possibly requiring surgery.
  • Hypercalcemia: In women with breast cancer and bone metastases, estrogen therapy may cause high calcium levels in the blood. If this occurs, the therapy should be stopped immediately.
  • Vision Problems: Estrogen therapy may cause blood clots in the eye, leading to vision problems. Stop the therapy if sudden vision changes occur.
  • Progestogen Addition: Adding progestogen to estrogen therapy can reduce the risk of endometrial hyperplasia, which can lead to cancer. However, it may increase breast cancer risk.
  • Blood Pressure: In rare cases, estrogen therapy may cause high blood pressure. Regular monitoring is advised.
  • High Triglycerides: Estrogen therapy can elevate triglyceride levels in the blood, possibly leading to pancreatitis. Monitor triglyceride levels during treatment.
  • Liver Problems: Estrogen may not be processed well by women with liver issues, leading to complications. Caution is needed, especially if there is a history of jaundice.
  • Thyroid Issues: Estrogen therapy may affect thyroid hormone levels. Women on thyroid medication may need adjustments, so thyroid function should be monitored.
  • Fluid Retention: Estrogen can cause water retention, leading to swelling. Women with heart or kidney issues should be closely monitored.
  • Low Calcium Levels: Estrogen may cause low calcium levels in women with hypoparathyroidism. Assess the risks and benefits of therapy.
  • Endometriosis Worsening: Estrogen therapy might worsen endometriosis, with rare cases of cancerous changes. Adding progestogen may help.
  • Hereditary Angioedema: Estrogen therapy may worsen symptoms of hereditary angioedema, a condition causing swelling. Evaluate the risks before starting therapy.
  • Other Conditions: Estrogen therapy may worsen conditions like asthma, diabetes, epilepsy, migraines, and lupus. Weigh the benefits and risks carefully.
  • Laboratory Tests: Certain blood tests may be affected by estrogen therapy. Monitoring is necessary for adjusting treatment as needed.

Overdosage

If you take too much Climara, you might experience symptoms like nausea, vomiting, tender breasts, stomach pain, feeling very tired, and unexpected bleeding.

If an overdose happens, stop using Climara right away and get medical help to treat these symptoms.

Drug Interactions With Other Medications

Certain medications can affect how Climara works because they interact with the way your body processes estrogen.

For example, some drugs and herbal supplements, like St. John’s wort, phenobarbital, carbamazepine, and rifampin, can make Climara less effective by reducing estrogen levels in your blood.

This might change how well the treatment works or affect your bleeding pattern.

On the other hand, medications like erythromycin, clarithromycin, ketoconazole, itraconazole, ritonavir, and even grapefruit juice can increase estrogen levels in your blood, which might cause side effects.

Alternative Medications

  • Vivelle-Dot
  • Estrace
  • Activella
  • Prempro
  • Gabapentin

While using Climara, avoid applying the patch to the breast or on oily, broken, or irritated skin. Do not place the patch on areas where it might be easily rubbed off, such as the waistline.
