What is Combivent?
Combivent (ipratropium bromide and albuterol) is a medication used to treat chronic obstructive pulmonary disease (COPD), a condition that makes it hard to breathe.
It combines two medicines: ipratropium bromide and albuterol sulfate.
Ipratropium bromide helps open the airways by blocking certain chemicals that cause them to constrict, while albuterol sulfate relaxes the muscles around the airways to make breathing easier.
This combination is used when a single medication isn’t enough to control symptoms.
However, it should not be used if you are allergic to any of its ingredients or to atropine and similar substances.
How Does Combivent Work?
Combivent works by combining two different types of medicines to help people with chronic obstructive pulmonary disease (COPD) breathe easier.
It contains ipratropium bromide, which blocks a chemical in the lungs that can cause the airways to tighten, and albuterol sulfate, which relaxes the muscles around the airways.
Together, these medicines help to open the airways more effectively than either one alone. Ipratropium prevents the airways from tightening by blocking certain receptors, while albuterol increases a substance in the cells that helps relax the muscles, making it easier to breathe.
This combination provides better control of breathing problems by targeting different pathways in the lungs.
Dosage Information and Usage
Dosage Form and Strengths of Combivent
Combivent comes as an inhaler with a cartridge.
Each inhaler dose delivers:
- 20 mcg ipratropium bromide (monohydrate)
- 100 mcg albuterol (equivalent to 120 mcg albuterol sulfate)
Usage
The recommended dosage of Combivent is one inhalation four times a day.
Patients may use additional inhalations if needed, but should not exceed a total of six inhalations in 24 hours.
Before using the inhaler for the first time, it must be primed by inserting the cartridge and actuating the inhaler toward the ground until an aerosol cloud is visible, then repeating this process three more times.
If the inhaler has not been used for more than 3 days, one actuation is needed to prepare it for use.
For inhalers not used for more than 21 days, actuate until an aerosol cloud is visible and repeat the priming process three additional times.
Storage Information
Store at room temperature, between 20°C to 25°C (68°F to 77°F).
It can be kept at temperatures ranging from 15°C to 30°C (59°F to 86°F) for short periods. Avoid freezing the inhaler.
Keep it out of reach of children and do not spray it into your eyes. After using the inhaler for 3 months or when the dose indicator shows that no more doses are left, discard it.

Warnings and Precautions
- Paradoxical Bronchospasm: Combivent can cause sudden worsening of breathing problems. If this happens, stop using the medication immediately and seek alternative treatment.
- Cardiovascular Effects: Albuterol sulfate in Combivent may lead to significant heart-related issues like changes in pulse rate, blood pressure, or other symptoms. Monitor for these effects, especially in people with heart conditions, and discontinue use if necessary.
- Ocular Effects: Ipratropium bromide can increase eye pressure, worsening narrow-angle glaucoma. Avoid getting the spray in your eyes to prevent eye pain, blurred vision, or other discomfort. Seek medical advice if these symptoms occur.
- Urinary Retention: Ipratropium bromide may cause difficulty with urination. Use with caution in patients with prostate problems or bladder obstructions.
- Do Not Exceed Recommended Dose: Overuse of inhaled sympathomimetic drugs can be dangerous and potentially fatal. Always adhere to the prescribed dose to avoid severe complications.
- Hypersensitivity Reactions Including Anaphylaxis: Allergic reactions such as rash, swelling, or difficulty breathing can occur. Stop using the medication immediately and seek medical help if these symptoms develop.
- Coexisting Conditions: Use Combivent with caution in individuals with convulsive disorders, hyperthyroidism, diabetes, or those sensitive to sympathomimetic drugs.
- Hypokalemia: Beta2-adrenergic agonists can lower potassium levels in the blood, which might affect heart function. This drop is usually temporary and does not require potassium supplements.
Overdosage
If you use too much Combivent, the main problems are likely to come from the albuterol sulfate.
Overusing albuterol can lead to serious issues like chest pain, high blood pressure, low potassium levels, fast heartbeat, and changes in blood chemistry.
In severe cases, it can even cause heart problems or death. If an overdose occurs, stop using Combivent immediately and seek medical help.
Treatment usually involves stopping the medication and getting medical care.
Dialysis is not effective for albuterol overdose, but a doctor may use a medication to manage heart issues.
Drug Interactions With Other Medications
- Anticholinergic Agents: Avoid using Combivent with other anticholinergic medications, as it may increase side effects like dry mouth or urinary retention.
- Beta-adrenergic Agonists: Be cautious when combining Combivent with other beta-agonists, as this can increase the risk of heart problems.
- Beta-receptor Blocking Agents: Beta-blockers can reduce the effectiveness of Combivent, so use them carefully, especially in people with sensitive airways.
- Diuretics: Combining Combivent with non-potassium-sparing diuretics might worsen changes in heart rhythm or low potassium levels; monitor potassium levels if needed.
- Monoamine Oxidase Inhibitors or Tricyclic Antidepressants: Use Combivent with caution in patients taking these medications or those who have stopped them within the last 2 weeks, as it can increase cardiovascular side effects. Consider alternative treatments if necessary.
