What is Effexor XR?

Effexor XR (venlafaxine) is a prescription medication approved by the FDA. It is used to treat several mental health conditions in adults. Specifically, it helps manage:

  • Major depressive disorder (MDD): This is a type of depression that causes persistent feelings of sadness and loss of interest in activities.
  • Generalized anxiety disorder (GAD): This condition involves excessive, uncontrollable worry about various aspects of life.
  • Social anxiety disorder (SAD): This disorder is marked by intense fear and avoidance of social situations due to concerns about being judged or embarrassed.
  • Panic disorder (PD): This involves sudden, recurrent panic attacks, which are episodes of intense fear and discomfort.

Effexor XR is not approved for use in pediatric patients. Limited studies have shown an increased risk of suicidal thoughts and behavior in children and adolescents taking antidepressants like Effexor XR.

Before starting Effexor XR, it is crucial to inform your healthcare provider if you have any medical conditions, such as a history of suicide, high blood pressure, heart issues, or liver problems.

It is also important to let them know if you are pregnant or breastfeeding, as the medication can affect your baby.

You should avoid taking Effexor XR if you are allergic to any of its ingredients or are currently using certain other medications, such as Monoamine Oxidase Inhibitors (MAOIs).

Effexor XR may interact with other medications and can cause serious side effects if stopped suddenly.

How Does Effexor XR Work?

Effexor XR works by increasing levels of two brain chemicals, serotonin and norepinephrine, which help improve mood and reduce anxiety.

It does this by preventing these chemicals from being reabsorbed back into nerve cells, making more of them available in the brain.

Effexor XR’s active ingredient, venlafaxine, also has a slight effect on dopamine, another mood-related chemical, but has minimal impact on other brain receptors that could cause side effects.

Dosage Information and Usage

Dosage Form and Strengths of Effexor XR

  • 37.5 mg extended-release capsule: Grey cap and peach body.
  • 75 mg extended-release capsule: Peach cap and body.
  • 150 mg extended-release capsule: Dark orange cap and body.

Usage

Administration:

  • Take Effexor XR once daily, either in the morning or evening, with food.
  • Swallow the capsules whole with water. Do not divide, crush, chew, or dissolve them.
  • If you have trouble swallowing capsules, open the capsule and sprinkle the contents on a spoonful of applesauce. Swallow the mixture immediately without chewing, and follow with a glass of water.

Starting Dose:

  • For Major Depressive Disorder (MDD) and Generalized Anxiety Disorder (GAD): Begin with 75 mg per day. The dose can be adjusted in increments up to 225 mg per day if needed.
  • For Social Anxiety Disorder (SAD): Start with 75 mg per day. Higher doses are not proven to be more effective.
  • For Panic Disorder (PD): Start with 37.5 mg per day for 7 days, then increase to 75 mg per day, up to a maximum of 225 mg per day if needed.

Adjustments:

  • Hepatic Impairment: Reduce the dose by 50% for mild to moderate liver issues, and more for severe liver problems.
  • Renal Impairment: Reduce the dose by 25% to 50% for mild to moderate kidney issues, and more for severe cases.

Discontinuation:

  • Gradually reduce the dose rather than stopping abruptly. The tapering schedule should be individualized; decreases of 37.5-75 mg every few days to weeks are often recommended based on tolerance. In some cases, tapering might take several months.
  • Switching Medications:
    • Allow at least 14 days between stopping an MAOI antidepressant and starting Effexor XR, and at least 7 days between stopping Effexor XR and starting an MAOI.

What to Avoid:

  • Do not drive or operate heavy machinery until you know how Effexor XR affects you, as it can cause drowsiness.
  • Avoid drinking alcohol, as it can increase the risk of serious side effects.

Warnings and Precautions

  • Suicidal Thoughts and Behaviors in Adolescents and Young Adults : Antidepressants, including Effexor XR, may increase the risk of suicidal thoughts and behaviors in patients aged 24 years and younger. This risk is most pronounced in those with major depressive disorder (MDD). Monitor young patients closely for any worsening of depression or emergence of suicidal thoughts, especially during the first few months of treatment and after dosage changes.
  • Serotonin Syndrome : Effexor XR can cause serotonin syndrome, a potentially life-threatening condition, especially when used with other serotonergic drugs or MAO inhibitors. Symptoms may include agitation, hallucinations, tremor, and severe autonomic instability. Discontinue Effexor XR immediately if serotonin syndrome is suspected and provide supportive care.
  • Elevated Blood Pressure : Effexor XR may cause increases in blood pressure, with some patients experiencing sustained hypertension. Monitor blood pressure before and during treatment, and manage pre-existing hypertension before starting Effexor XR. Consider dose adjustment or discontinuation if significant increases in blood pressure occur.
  • Increased Risk of Bleeding : Effexor XR may increase the risk of bleeding, especially when used with other anticoagulants or drugs affecting platelet function. Inform patients about this risk and monitor those on anticoagulants closely. Be cautious with the use of nonsteroidal anti-inflammatory drugs (NSAIDs) and aspirin.
  • Angle-Closure Glaucoma : Use of Effexor XR may trigger angle-closure glaucoma in patients with anatomically narrow angles. Avoid using Effexor XR in such patients unless they have undergone corrective surgery. Monitor for signs of glaucoma if treatment is necessary.
  • Activation of Mania or Hypomania : Effexor XR may trigger manic or hypomanic episodes in patients with bipolar disorder. Screen for a personal or family history of bipolar disorder before starting treatment. Monitor for any signs of mania or hypomania during treatment.
  • Discontinuation Syndrome : Abrupt discontinuation or rapid dose reduction of Effexor XR can lead to symptoms such as agitation, dizziness, and nausea. Gradually taper the dose to minimize discontinuation symptoms and monitor patients closely. Severe symptoms, including suicidal thoughts, may require resuming the medication at a lower dose.
  • Seizures : Effexor XR may increase the risk of seizures, particularly in patients with a history of seizure disorders. Prescribe with caution in such patients and monitor for any seizure activity. Discontinue Effexor XR if seizures occur.
  • Hyponatremia : Effexor XR can cause low sodium levels in the blood, often due to the Syndrome of Inappropriate Antidiuretic Hormone (SIADH). Monitor for symptoms of hyponatremia, especially in elderly patients or those on diuretics. Discontinue treatment if severe hyponatremia develops and provide appropriate medical intervention.
  • Weight and Height Changes in Pediatric Patients : Effexor XR has been associated with weight loss in pediatric patients. The effects on height are not well established. Monitor weight regularly in children and adolescents if treatment is considered off-label.
  • Appetite Changes in Pediatric Patients : Decreased appetite, or anorexia, has been reported more frequently in pediatric patients taking Effexor XR compared to those on placebo. Monitor for changes in appetite and growth in pediatric patients. Effexor XR is not approved for use in pediatric patients.
  • Interstitial Lung Disease and Eosinophilic Pneumonia : Rare cases of interstitial lung disease and eosinophilic pneumonia have been associated with Effexor XR. Patients presenting with new or worsening respiratory symptoms should be evaluated promptly. Discontinue Effexor XR if these conditions are suspected.
  • Sexual Dysfunction : Effexor XR can cause sexual side effects, including decreased libido and delayed orgasm. Discuss potential sexual side effects with patients before starting treatment and monitor for these symptoms. Address any concerns about sexual function and consider alternative treatments if needed.

Overdosage of Effexor XR

Overdosing on Effexor XR can be serious and requires immediate medical attention.

Common symptoms of an overdose include drowsiness, dizziness, nausea, and sensations of numbness or tingling in the limbs.

In severe cases, it may lead to more serious issues like seizures, changes in heart rhythm, and even coma. Treatment typically involves hospitalization, where doctors may use activated charcoal to limit further absorption of the drug and provide supportive care.

There is no specific antidote for Effexor XR overdose, so managing the condition often involves treating the symptoms and monitoring the patient closely.

Drug Interactions With Other Medications

  • Monoamine Oxidase Inhibitors (MAOIs): Using Effexor XR with MAOIs increases the risk of serotonin syndrome, a serious condition with symptoms like agitation and confusion. Effexor XR should not be used with MAOIs, including linezolid or intravenous methylene blue.
  • Other Serotonergic Drugs: Combining Effexor XR with other serotonergic drugs (e.g., SSRIs, triptans, opioids) raises the risk of serotonin syndrome. Monitor for symptoms and consider stopping Effexor XR or the other serotonergic drugs if serotonin syndrome occurs.
  • Drugs that Interfere with Hemostasis: Using Effexor XR with antiplatelet or anticoagulant drugs can increase bleeding risk due to its effect on serotonin release by platelets. Monitor patients closely for bleeding if they are on these medications.
  • Effect of CYP3A Inhibitors: CYP3A inhibitors can increase levels of venlafaxine and its active form, raising the risk of toxicity. Reducing the dose of Effexor XR may be necessary when used with these inhibitors.
  • CYP2D6 Substrates: Effexor XR can increase the levels of drugs metabolized by CYP2D6, potentially leading to higher toxicity. Adjusting the dose of these CYP2D6 substrates may be required.
  • Central Nervous System (CNS)-Active Drugs: Combining Effexor XR with other CNS-active drugs (including alcohol) hasn’t been well-studied, so caution is advised. Be aware of potential interactions and adverse effects when using these drugs together.
  • Weight Loss Agents: Effexor XR is not recommended for use with weight loss agents like phentermine, as its safety and effectiveness in this combination have not been established. Effexor XR should not be used for weight loss.
  • Laboratory Test Interference: Venlafaxine can cause false-positive urine tests for phencyclidine (PCP) and amphetamines. Confirmatory tests, like gas chromatography/mass spectrometry, are needed to distinguish venlafaxine from these substances.

Frequently Asked Questions

Effexor XR is a prescription medication used to treat major depressive disorder, anxiety disorders, and panic disorder. It is an extended-release formulation of venlafaxine, which helps balance certain chemicals in the brain to improve mood and reduce anxiety.
Effexor XR works by increasing the levels of neurotransmitters, serotonin and norepinephrine, in the brain. This helps improve mood, relieve anxiety, and alleviate symptoms of depression. The extended-release formulation ensures a steady release of the medication over time.
The most important information about Effexor XR is that it can increase the risk of serotonin syndrome, especially if taken with other serotonergic drugs. It’s crucial to follow the prescribed dosage and consult your doctor before stopping or changing your medication. It may increase blood pressure, so regular monitoring is recommended.
Take Effexor XR once a day with food. You can take it either in the morning or evening, but stick to the same time each day. Swallow the capsule whole with a drink. Your doctor may adjust your dose to find the right amount for you.
Yes, Effexor XR should be taken every day, typically once per day. The exact dosage may vary based on your condition.
To discontinue Effexor XR, follow your doctor’s instructions carefully. Typically, you should reduce your daily dose by 37.5 to 75 mg each week for about four weeks. This gradual reduction helps minimize withdrawal symptoms.
Possible side effects of Effexor XR include nausea, dizziness, dry mouth, insomnia, and increased sweating. You might also experience fatigue, changes in appetite, or sexual dysfunction.
Effexor XR should not be taken with MAO inhibitors, as this can lead to a serious and potentially fatal drug interaction. Avoid using MAO inhibitors such as isocarboxazid, linezolid, and phenelzine while on Effexor XR.
If you miss a dose of Effexor XR, take it as soon as you remember, unless it is almost time for your next dose. Do not double up on doses to make up for a missed one.
You should avoid or limit alcohol while taking Effexor XR. Alcohol can increase the risk of side effects and affect your mental alertness.
Effexor XR may start to improve symptoms within 1 to 2 weeks. However, it typically takes 4 to 8 weeks to experience the full benefits of the medication.
Effexor XR is not considered addictive. However, stopping it suddenly can lead to withdrawal symptoms like dizziness, sleep problems, irritability, nausea, and headaches.
Yes, Effexor XR can cause weight gain as a long-term side effect. While some people may experience weight loss instead, it’s important to monitor any changes in weight and discuss them with your doctor. Long-term use of Effexor XR may increase the risk of various side effects, including weight gain.
Effexor XR can be used during pregnancy, but it should only be taken if the benefits outweigh the risks.
If you experience mild symptoms of an allergic reaction to Effexor XR, contact your doctor immediately. They will advise on managing symptoms and whether you should continue taking the medication.
Effexor XR is not specifically approved for treating insomnia. It is primarily prescribed for depression, anxiety, and other mood disorders. However, if you have insomnia as a symptom of a broader condition being treated with Effexor XR, it may help improve sleep indirectly by addressing the underlying issue.
Effexor XR is not approved by Health Canada for use in children and adolescents. Its use in this age group is not recommended unless specifically advised by a healthcare provider.
If you experience severe side effects from Effexor XR, seek medical help immediately. Symptoms to watch for include fast heartbeat, hallucinations, loss of coordination, severe dizziness, severe nausea, vomiting, diarrhea, muscle twitching, unexplained fever, and unusual agitation. Although rare, a very serious allergic reaction may also occur.
Effexor XR can interact with herbal supplements, such as St. John’s wort, which is used for depression and other conditions. Combining Effexor XR with St. John’s wort can increase the risk of serotonin syndrome. It is advisable to consult your doctor before using herbal supplements with Effexor XR.
Effexor XR should not be taken by children and young adults under the age of 24 due to an increased risk of suicidal thoughts and behavior. This risk is associated with all antidepressants, not just Effexor XR.
