What is Erleada?

Erleada (apalutamide) is a prescription medicine used to treat certain types of prostate cancer.

It is effective for two main conditions: metastatic castration-sensitive prostate cancer (mCSPC), where cancer has spread to other parts of the body but still responds to treatments that lower testosterone, and non-metastatic castration-resistant prostate cancer (nmCRPC), where cancer has not spread but no longer responds to testosterone-lowering treatments.

Erleada helps manage these cancers by blocking the effects of testosterone, a hormone that can fuel prostate cancer growth.

Before starting Erleada, inform your healthcare provider about any existing health issues, such as heart disease, high blood pressure, diabetes, or a history of seizures or brain problems.

If you are pregnant, planning to become pregnant, or breastfeeding, discuss this with your doctor, as Erleada may impact pregnancy and might pass into breast milk. Men with partners who can become pregnant should use effective birth control during treatment and for three months afterwards.

How Does Erleada Work?

Erleada works by blocking the action of testosterone, a hormone that can fuel the growth of prostate cancer. It contains an ingredient called apalutamide, which targets and attaches to the androgen receptor (AR) in cancer cells.

By doing this, Erleada prevents the androgen receptor from moving to the cell nucleus, where it normally helps cancer cells grow and divide.

This blockage stops the cancer cells from receiving signals that they need to grow.

As a result, Erleada helps reduce tumor growth and can lead to smaller tumors and fewer cancer cells.

In clinical studies, Erleada has been shown to lower prostate-specific antigen (PSA) levels, which is a marker used to monitor prostate cancer, more effectively when used with other treatments compared to those treatments alone.

Dosage Information and Usage

Dosage Form and Strengths of Erleada

  • 240 mg Tablets: Bluish grey to grey, oval, film-coated, and marked with “E240”.
  • 60 mg Tablets: Slightly yellowish to greyish green, oblong, film-coated, and marked with “AR 60”.

Usage

Erleada should be taken once daily, either as one 240 mg tablet or four 60 mg tablets. Swallow the tablets whole without crushing or splitting them.

It can be taken with or without food. Erleada can be dispersed in non-carbonated water and taken with orange juice, applesauce, or additional water for those who have difficulty swallowing tablets.

It can also be administered through a feeding tube if needed. Patients should also receive a gonadotropin-releasing hormone (GnRH) analog or have had a bilateral orchiectomy (surgical removal of the testicles).

If a dose is missed, take it as soon as possible on the same day but do not take extra tablets to make up for the missed dose.

Storage Information

Store Erleada at room temperature, between 68°F and 77°F (20°C to 25°C). Keep it in its original container to protect it from light and moisture.

The bottle includes a desiccant packet, which helps keep the medicine dry. Make sure not to discard the desiccant, as it is important for preserving the quality of your medication.

Overdosage

If you take too much Erleada, there is no specific antidote available. In case of an overdose, stop taking Erleada immediately.

Seek medical attention and follow general supportive care to manage any symptoms until the effects of the overdose lessen or resolve.

Your doctor will guide you on the best course of action to ensure your safety and recovery.

Drug Interactions With Other Medications

  • Strong CYP2C8 or CYP3A4 Inhibitors: Taking a strong inhibitor of CYP2C8 or CYP3A4 with Erleada may increase the levels of Erleada in your body. While no initial dose adjustment is required, your doctor might reduce the Erleada dose based on how you tolerate the medication.
  • Mild or Moderate CYP2C8 or CYP3A4 Inhibitors: These are not expected to affect Erleada levels significantly, so no special precautions are usually needed.
  • CYP3A4, CYP2C9, CYP2C19, and UGT Substrates: Erleada can reduce the effectiveness of medications metabolized by CYP3A4, CYP2C19, or CYP2C9. If you are taking these medications, your doctor might suggest alternatives or monitor for reduced effectiveness.
  • UGT Substrates: Erleada can also lower the levels of drugs processed by UDP-glucuronosyl transferase (UGT). If you need to take these drugs, use caution and check for reduced activity.
  • P-gp, BCRP, or OATP1B1 Substrates: Erleada may decrease the levels of drugs that are substrates for P-glycoprotein (P-gp), breast cancer resistance protein (BCRP), or organic anion transporting polypeptide 1B1 (OATP1B1). Be cautious if these drugs are used together and monitor for reduced effectiveness.

Alternative Medications
