What is Estalis?

Estalis (estradiol/norethindrone acetate) is a medication designed for women who are experiencing menopause and still have their uterus.

Menopause can cause uncomfortable symptoms like hot flashes, night sweats, and vaginal issues such as dryness, itching, or burning. Estalis helps to relieve these symptoms.

Because Estalis involves hormone replacement, it’s important to use it under the guidance of a doctor.

Before starting Estalis, it’s important to discuss the risks and benefits with your doctor. Not everyone can use this medication.

It’s not suitable if you are pregnant, breastfeeding, or if you have a history of certain cancers, blood clots, heart disease, liver problems, or migraines.

How Does Estalis Work?

Estalis works by releasing two hormones into your body through a patch you wear on your skin.

The first hormone, estradiol, is a type of estrogen that helps to relieve menopause symptoms like hot flashes and vaginal dryness.

The second hormone, norethindrone acetate (NETA), is a type of progestin that works to protect the lining of your uterus.

Together, these hormones help balance the changes your body goes through during menopause, making you feel more comfortable.

Dosage Information and Usage

Dosage Form and Strengths of Estalis

Estalis is available as a transdermal patch that is applied to the skin.

Each pack contains 8 patches, and there are two strengths available.

Your doctor will decide the appropriate strength based on your specific needs.

  • Estalis 140/50:
    • Patch size: 9 cm²
    • Releases: 50 micrograms of estradiol and 140 micrograms of norethindrone acetate per day
  • Estalis 250/50:
    • Patch size: 16 cm²
    • Releases: 50 micrograms of estradiol and 250 micrograms of norethindrone acetate per day

Usage

Estalis patches are typically used in a continuous regimen, meaning you wear them all the time. You apply a new patch twice a week, on the same two days each week.

Each patch should stay on for 3 to 4 days before being replaced with a new one.

The patch should be applied to clean, dry skin on your abdomen or buttocks, avoiding areas where clothing might rub it off.

It’s important to rotate the application site each time you put on a new patch to avoid skin irritation.

Do not apply the patch to your breasts, as this can cause unwanted side effects.

Always follow your doctor’s instructions on how to use the patches and do not change or stop your treatment without consulting your doctor.

Storage Information

Store Estalis patches at room temperature (20-25°C) and use them within 6 months of purchase or by the expiry date on the pack, whichever comes first.

You can also keep them in the refrigerator (2-8°C) and use them before the expiry date.

Let the patches reach room temperature before applying them.

Do not freeze the patches. Keep them in their original package and make sure they are out of the reach of children and pets.

Warnings and Precautions

  • Breast Cancer: Women with a personal or family history of breast cancer or abnormal mammograms should consult their doctor before starting hormone replacement therapy (HRT). Regular breast exams and mammograms are recommended.
  • Overgrowth of the Uterus Lining (Endometrial Hyperplasia) and Uterus Cancer: Estrogen therapy alone can increase the risk of endometrial hyperplasia, which may lead to endometrial cancer. Adding progestin can reduce this risk. Women should report any unusual vaginal bleeding to their doctor.
  • Ovarian Cancer: Long-term use of estrogen or estrogen plus progestin therapy (5 years or more) may increase the risk of ovarian cancer.
  • Heart Disease and Stroke: HRT may increase the risk of heart disease and stroke, particularly in postmenopausal women. The risk varies depending on the type of HRT and whether the woman has had a hysterectomy.
  • Abnormal Blood Clotting: The risk of blood clots in the lungs and large veins increases with HRT, especially in older women, smoke, are overweight, or have a family history of blood clots.
  • Gallbladder Disease: Estrogen use in postmenopausal women has been linked to an increased risk of gallbladder disease, which may require surgery.
  • Dementia: Women over 65 using oral combined estrogen plus progestin may have a higher risk of dementia (memory loss). This risk was not observed in women using estrogen alone.
  • Severe Allergic Reactions: If you have a history of severe allergic reactions or hereditary angioedema (swelling of hands, feet, or throat), discuss this with your doctor before starting treatment.
  • Unexplained Vaginal Bleeding: Any unexplained or unusual vaginal bleeding should be reported to your doctor immediately, as it may indicate a serious condition.
  • Liver Disease: Women with a history of liver disease, jaundice, or liver tumors should be cautious when using estrogen and should discuss this with their doctor.
  • Migraine Headaches: If you have a history of migraines, especially with aura (visual disturbances), consult your doctor, as HRT may worsen these symptoms.
  • High Blood Pressure: Women with a history of high blood pressure should monitor their blood pressure regularly while using HRT.
  • Blood Clots and Cardiovascular Conditions: If you have a history of blood clots, heart disease, or stroke, or if you smoke, your risk of these conditions may increase with HRT.
  • Kidney Disease or Asthma: Discuss with your doctor if you have a history of kidney disease or asthma, as HRT may affect these conditions.
  • Bone Disease: If you have bone disease, particularly those that affect calcium and phosphorus levels, consult your doctor before starting HRT.
  • Diabetes: Women with diabetes should monitor their blood sugar levels, as HRT may affect glucose control.
  • Porphyria: If you have been diagnosed with porphyria (a blood disorder), HRT may affect this condition.
  • Lupus: Women with lupus should be cautious with HRT, as it may exacerbate symptoms.
  • Depression: Women with a history of depression should monitor their mood, as HRT may affect mental health.
  • Hearing Loss: Women with hearing loss due to otosclerosis (a condition affecting the bones in the ear) should consult their doctor.
  • Epilepsy or Neurological Disorders: If you have epilepsy or other neurological conditions, HRT may affect seizure control.
  • High Cholesterol or Triglycerides: Women with a history of high cholesterol or triglycerides should monitor their lipid levels during HRT.
  • Pregnancy and Breastfeeding: HRT should not be used if you are pregnant or breastfeeding.
  • Hysterectomy: If you have had a hysterectomy (removal of the uterus), discuss with your doctor whether you need progestin in addition to estrogen.
  • Smoking: Smoking increases the risk of serious side effects from HRT, including blood clots, heart disease, and stroke.
  • Surgery or Long Bed Rest: HRT may increase the risk of blood clots during periods of immobilization, such as after surgery or long bed rest. Discuss preventive measures with your doctor.
  • Hypothyroidism: If you have hypothyroidism and are on thyroid hormone replacement therapy, your doctor may need to monitor and adjust your treatment during HRT.

Overdosage of Estalis

If you use too much Estalis, you might experience symptoms like nausea, sore breasts, swelling, bloating, or vaginal bleeding.

Using too much of the progestin part of the patch can lead to feeling sad, tired, acne, or more hair growth than usual.

If you think you’ve used too much, take off the patch and contact your doctor, the nearest emergency department, or a poison control center right away.
