What is Eylea?

Eylea (aflibercept) is an eye medication for various eye diseases involving either loss of vision or vision impairment.

Eylea inhibits the pathological processes of neovascular or wet age-related macular degeneration, which is a blurring or distortion of central vision brought about by older ages, and macular edema resulting from such diseases as retinal vein occlusion (RVO) or diabetic macular edema (DME).

Eylea also treats diabetic retinopathy (DR), a complication of diabetes, which severely damages blood vessels in the eye, and ROP, affecting the eyes of premature babies.

Always follow the instructions given by your doctor as you would not use this medication if you have an infection of the eye, active inflammation inside the eye, or an allergy to one of the ingredients.

How Does Eylea Work?

Eylea works by affecting abnormal blood vessel formation and prevents fluid from leaking into the eye through proteins that exist in the eye.

Most of the time, these proteins, VEGF-A and PlGF, drive new vessel growth; excessive stimulation of these proteins leads to problems, through edema or the growth of new blood vessels in sites that would not be considered appropriate within the eye.

Eylea acts as a soluble decoy receptor that binds vascular endothelial growth factor-A (VEGF-A) and placental growth factor (PlGF). By blocking these proteins, it prevents abnormal blood vessel growth and fluid leakage in the retina, helping to slow or prevent vision loss.

Dosage Information and Usage

A trained physician administers the injections using a specially calibrated syringe. The recommended dose is 2 mg (0.05 mL of 40 mg/mL solution) administered by intravitreal injection. Dosing intervals vary depending on the condition. For example, every 4 weeks initially for wet AMD, followed by less frequent dosing as directed by the physician.

Dosage Form and Strengths of Eylea

Eylea is supplied in a prefilled syringe and a vial. Irrespective of the one chosen, the concentration of the drug within is 40 mg/mL of aflibercept.

A prefilled syringe is provided that contains more than the required 2 mg dose. Thus, it should be disposed of before administration.

This drug will be drawn up using the vial of needles and syringes that also contain aflibercept, using some specific needles and syringes.

Usage

  • Prepare: Open the Eylea carton and tear open the sterile blister pack. Store the syringe in the sterile tray until ready to assemble.
  • Remove syringe: Using the aseptic technique, withdraw the syringe from the blister pack.
  • Twist off syringe cap: Twist off the syringe cap, instead of snapping it off.
  • Attach needle: Attach a 30-gauge by ½-inch injection needle to the syringe by twisting it onto the Luer lock.
  • Remove air bubbles: Hold the syringe upright and tap it to loosen air bubbles in the line.
  • Inject the dose: Expel air bubbles and adjust the syringe to a volume of 0.05 mL (50 microliters), which equals 2 mg of aflibercept.
  • Administer: Under sterile conditions, the drug is injected into the eye. The unused product is disposed of after injection.

Storage Information

Store the solution for Injection in a refrigerator between 2°C to 8°C (36°F to 46°F). Do not freeze. The expiry date is on the carton and container label.

Store Eylea in its original carton until the time of injection. Avoid exposure to light. Do not open the sealed blister pack of medication until you are ready to administer the drug.

Side Effects of Eylea

As with any medication, side effects can also be experienced with Eylea. That is why you should learn which of them exist and discuss them with your doctor if you do start noticing unfamiliar symptoms.

Common Side Effects

  • Eye pain
  • Conjunctival hemorrhage (bleeding in the eye)
  • Cataracts
  • Vitreous detachment (eye condition)
  • Vitreous floaters (small spots in vision)
  • Increased intraocular pressure (pressure in the eye)

Serious but Rare Side Effects

  • Retinal detachment
  • Endophthalmitis (eye infection)
  • Retinal vasculitis (inflammation of the retina)
  • Increased risk of blood clots (thromboembolic events)

Warnings and Precautions

Eylea is an ophthalmic injection for the specified conditions, and there are, as in every other course of treatment, risks and precautions to be known by the patient as well as those concerned with the patient’s care. Some important warnings and precautions are listed below:

  • Risk of Endophthalmitis and Retinal Detachments: Eylea, like intracameral and intraocular injections, carries a risk of endophthalmitis or retinal detachment. Strict injection techniques must be performed. All signs, such as ocular pain and vision loss, should be reported immediately.
  • Retinal Vasculitis with or without Occlusion: At times, the tiny vessels within the eye become inflamed with or without occlusion. When this happens, it calls for immediate medical care.
  • Increased Eye Pressure: The intraocular pressure may transiently elevate within an hour or so of injection and should be carefully monitored by the physicians and controlled as required.
  • Extended Follow-up in ROP Infected Infants: Following the treatment of Eylea for Retinopathy of Prematurity, extended follow-up is required in the infected infants in case the disease may recur, and the abnormal blood vessels begin to grow back again.
  • Blood Clots (Thromboembolic Events): Blood clots in the legs or lungs that may lead to death or stroke were seen in some of the patients receiving Eylea. These serious blood clots can occur within days of starting treatment, and the risk is increased with certain conditions.

Overdosage of Eylea

Even a minimum overdose can increase the pressure within the eye, leading to too much Eylea injected into the eye.

The doctor has to measure the pressure in the eye for appropriate treatment.

Primarily, monitoring and swift response to overdosing can avoid many complications caused by the overdose of Eylea.

Alternative Medications

Here is a list of alternative medications that can be used instead of Eylea for similar conditions:

  • Vabysmo (faricimab)
  • Syfovre (pegcetacoplan)
  • Lucentis (ranibizumab)
  • Vabysmo (faricimab)
  • Izervay (avacincaptad pegol)

Frequently Asked Questions

Eylea is an injective medication designed for the treatment of certain diseases of the eyes, for instance, age-related macular degeneration, diabetic macular edema, and retinal vein occlusion.
Eylea typically stays in the body for about 8 weeks after the initial injection. The recommended dosage is 2 mg administered by intravitreal injection every 4 weeks for the first 12 weeks, followed by 2 mg every 8 weeks thereafter.
After injection, do not rub the injected eye with Eylea; it may irritate or cause complications. You are also not allowed to wash your face and hair nor take a shower for at least 48 hours to prevent the eyes from getting water and soap into them. Finally, you will be required to shun swimming for one whole week simply to reduce infection levels.
Eylea typically begins to work within three months, although some individuals may notice improvements in as little as one month, depending on the specific condition being treated.
The discontinuation of the treatment appears to have a high probability of losing eyesight. When it is progressive, loss of eyesight may be from an acute cause by a large major bleed in the eye. Most patients deteriorate in conditions regarding eyes when the treatment is stopped.
Eylea is administered through an intravitreal injection, which means it is injected directly into the eye.
The treatment schedule of Eylea is at intervals of 4 weeks, or alternative, every 8 weeks depending upon the severity of your condition. To most, some type of treatment will continue to be administered after one year in a stretched interval of 12 weeks. With diabetic macular edema and neovascular age-related macular degeneration, Eylea HD injections are administered every 8 to 16 weeks, following initial 12 weeks of monthly injections.
Yes, Eylea has side effects. Known side effects include eye pain, redness, and increased intraocular pressure. Other possible side effects could be cataracts, retinal detachment and endophthalmitis; infection inside the eye. You must mention any unusual symptoms to your doctor.
Yes, allergic reactions may include severe; but in case you experience edema and are unable to breathe while having a rash too, simply cease application of Eylea and get medical help as soon as possible.
If you miss your scheduled Eylea injection, it’s important not to skip it. Make a new appointment as soon as possible to receive your injection.

Store Eylea in the refrigerator at 2°C to 8°C (36°F to 46°F). Store it in the original pack when not in use. Keep the medication away from direct and indirect light. Do not freeze. Do not administer frozen product. Assure that the product has not passed its expiration date prior to use of the product.
There is limited information on Eylea use in pregnancy and lactation. In women receiving the 8 mg of Eylea, highly effective contraception should be continued at least 4 months after the last injection. Low levels of Eylea are excreted into breast milk; its effects in neonates or infants are unknown.
Eylea’s safety and effectiveness in children are not known, as there is limited information available on its use in this age group.
If you notice vision change soon after receiving Eylea, be sure to see your doctor immediately. Vision change can be an indication of potential complications, which should be looked into in consultation with a health care provider.
You must talk to your physician about all health conditions, medications, vitamin and mineral, herbs or supplements before you begin Eylea. Your treatment with Eylea will depend on all this information after discussion with your treating provider. Talk about any of your medical conditions or treatments in your history that may have relevance for your treatment.
With any eye medication, precautions need to be followed. Thus, one needs to inform the doctor of all the medical conditions and medications one is taking. It requires proper aseptic techniques while administering the injection to reduce infection. In case of some change in vision or discomfort in the eyes, it should be followed up with the healthcare provider.
You may see floating objects in your vision following an injection of Eylea. Avoid driving or operating machinery until your vision has cleared.
One visit for an Eylea injection takes approximately 50 minutes total. Of course, if all your vision and OCT tests need to be done on the same day as your injection, this is much longer. The healthcare team prepares you for the injection and monitors you through the treatment process to ensure your safety and comfort during it.
Over-the-counter pain-relieving drugs like Tylenol can usually be used for mild to moderate eye irritation, and you can apply a cool compress or an ice pack to your eyes after having had the Eylea injection. Artificial tears or lubricating drops can also help to keep your eyes comforted and moist.

Usually, the pressure in the eye is felt, but the procedure itself causes little or no pain. Finally, having injected, the speculum, used to keep the eye open, is removed and the eye cleaned. The whole process takes 10-15 minutes. In most cases, severe complications with intravitreal injections seldom develop.
