What are Fiasp Cartridges?

Fiasp Cartridges are prefilled insulin cartridges containing 3 mL of rapid-acting insulin aspart at 100 units/mL (U-100). They are designed for use with refillable PenFill-compatible pens, such as the NovoPen series, and are not compatible with disposable prefilled pens like FlexTouch.

Fiasp’s formulation includes Vitamin B3 (niacinamide) for faster absorption and L-arginine for stability, allowing it to start working almost immediately after injection to manage mealtime blood sugar levels.

Important Information to Tell and Ask Your Doctor Before Taking Fiasp

• If you have trouble with your adrenal, pituitary, or thyroid glands, your doctor may need to alter your insulin dose.
• If you have an infection, fever, or have had an operation, you may need more insulin than usual.
• If you suffer from diarrhea, or vomiting, or are eating less than usual, you may need less insulin.
• If you exercise more than usual or plan to change your regular diet, inform your healthcare provider.
• If you are ill, continue taking your insulin as your need for it may change.
• If you plan to travel abroad, traveling over time zones may affect your insulin needs and the timing of your injections, so consult your doctor before such travel.
• If you are pregnant, planning a pregnancy, or breastfeeding, consult your doctor for advice.
• If you drive or use tools or machines, watch for signs of hypoglycemia, as your ability to concentrate or react will be reduced during a hypoglycemic reaction. Please keep this in mind in situations where you might put yourself and others at risk.

Dosage Information and Usage

Dosage Form and Strengths of Fiasp Cartridges

Fiasp is available as 3 mL PenFill cartridges with a concentration of 100 units/mL (U-100).

Usage

Fiasp insulin should be injected into the subcutaneous tissue, avoiding injecting into the same spot repeatedly to prevent the formation of lumps.

The recommended injection sites include the front of the thighs, the abdomen (front of the waist), or the upper arm, with injection around the waist for faster insulin action.

Regularly monitor blood glucose levels. Only adjust your insulin regimen as instructed by your doctor. When transitioning between insulin types or brands, your doctor may need to modify your dosage.

For best results, inject Fiasp up to 2 minutes before a meal. If needed, you can also take it up to 20 minutes after starting your meal, following your healthcare provider’s guidance.

Storage Information

Before opening, Fiasp PenFill should be kept in a refrigerator between 36°F to 46°F (2°C to 8°C) until the expiration date.

If stored outside of refrigerated conditions before opening, it can be kept at room temperature (below 86°F or 30°C) for up to 4 weeks.

Once opened and in use, Fiasp PenFill cartridges should not be refrigerated and must be stored at room temperature for a maximum of 4 weeks (28 days).

Common Side Effects

• Skin issues like eczema, rash, itching, redness, and swelling of the skin (dermatitis)
• Reactions at the injection site, like itching and rash
• Skin thickening or pits at the injection site (lipodystrophy)
• Weight gain

Serious but Rare Side Effects

• Low blood sugar (hypoglycemia), which may present with symptoms such as:
  • dizziness or light-headedness,
  • blurred vision,
  • anxiety,
  • sweating,
  • confusion,
  • shakiness,
  • headache,
  • fast heartbeat, and
  • hunger

• Low potassium in your blood (hypokalemia).
• Serious allergic reactions (whole-body reactions) are characterized by:
  • a rash over your entire body,
  • difficulty breathing,
  • rapid heartbeat,
  • swelling of your face, tongue, or throat, sweating,
  • extreme drowsiness, dizziness, or confusion.

• Heart failure, particularly when taking thiazolidinediones (TZDs) with Fiasp. Symptoms of heart failure may include
  • shortness of breath,
  • swelling of ankles or feet, and
  • sudden weight gain

Warnings and Precautions

• Fiasp FlexTouch pens, PenFill cartridges, and cartridge devices should not be shared among patients, even if the needle is changed. This also applies to Fiasp vials, needles, or syringes, as sharing can spread blood-borne pathogens.
• Modifying your insulin regimen, like changing the type or strength of insulin, injection site, or administration method, can impact your blood sugar levels. This might cause low (hypoglycemia) or high (hyperglycemia) blood sugar. Make any adjustments under careful medical guidance and monitor your blood glucose more frequently.
• Hypoglycemia is the main adverse reaction with Fiasp and all insulin treatments. Symptoms may include dizziness, blurred vision, anxiety, sweating, confusion, shakiness, headache, rapid heartbeat, and hunger. Severe hypoglycemia can cause seizures, loss of consciousness, or death. Be careful when driving or using machinery, as hypoglycemia can affect focus and reaction speed.
• Accidental mix-ups between different insulin products have been reported. Always check the insulin label before each injection to prevent medication errors.
• Fiasp and other insulin products may lower potassium levels, potentially causing severe issues like respiratory paralysis or irregular heartbeats. Monitor at-risk patients’ potassium levels and make adjustments as needed.
• Severe allergic reactions, including anaphylaxis, can occur with insulin products like Fiasp. Discontinue Fiasp if hypersensitivity reactions occur and treat according to standard care.
• Thiazolidinediones (TZDs), used with Fiasp, can cause fluid retention and potentially worsen heart failure. Monitor for signs and symptoms of heart failure if using both medications concurrently.
• Insulin pump or infusion set malfunctions can lead to rapid hyperglycemia and ketoacidosis. Promptly address pump issues and have a plan for interim subcutaneous insulin injection if necessary.

Overdosage

Taking too much Fiasp, delaying or having a smaller meal than usual, exercising more, or drinking alcohol can cause hypoglycemia, a drop in blood sugar.

Signs of hypoglycemia can appear suddenly and include sweating, pale skin, headache, slurred speech, rapid heartbeat, nausea, intense hunger, vision changes, drowsiness, weakness, trembling, anxiety, confusion, and difficulty concentrating.

Severe hypoglycemia can cause brain damage or death if not treated. If you lose consciousness from hypoglycemia or have frequent episodes, contact your doctor right away.

You might need changes to your insulin, meals, or exercise to avoid future episodes.

Drug Interactions

• Using Fiasp with medications such as antidiabetic agents, ACE inhibitors, or certain antibiotics like sulfonamides can heighten the risk of low blood sugar levels (hypoglycemia). It’s essential to monitor your glucose levels closely and potentially adjust your Fiasp dosage under medical supervision when taking these medications together.
• Some medications, like corticosteroids or certain hormonal therapies such as oral contraceptives, may reduce the effectiveness of Fiasp in lowering blood sugar levels. Adjustments in Fiasp dosage and increased monitoring of blood glucose levels may be necessary to maintain adequate control when these medications are used concurrently.
• Alcohol, beta-blockers, and other medications like lithium salts can either increase or decrease the blood glucose-lowering effects of Fiasp. Depending on the specific medication, dose adjustments and more frequent monitoring of blood sugar levels may be required to manage these interactions effectively.
• Certain medications such as beta-blockers or clonidine can mask or reduce the typical signs and symptoms of hypoglycemia, making it harder to recognize low blood sugar levels. When using these medications with Fiasp, it’s important to monitor blood glucose levels more frequently to ensure early detection and management of hypoglycemia episodes.
