What is Fragmin?

Fragmin (dalteparin) is a medication used to prevent and treat blood clots.

It helps reduce the risk of serious conditions like deep vein thrombosis (DVT) and pulmonary embolism (PE), which can occur after surgeries or in people with limited movement due to illness.

Fragmin is also used to manage venous thromboembolism (VTE) in people with cancer and children with VTE.

It is given as an injection and works by slowing down the blood clotting process.

However, Fragmin should not be used in people with active major bleeding, certain types of allergic reactions to heparin, or those undergoing specific types of anesthesia.

How Does Fragmin Work?

Fragmin works by helping to prevent blood clots. It contains a substance called dalteparin, which is a type of heparin.

Heparin is a blood thinner that affects how blood clots form.

Specifically, Fragmin enhances the action of proteins in the blood that block clotting factors, mainly Factor Xa and thrombin.

Factor Xa and thrombin are key factors in the clotting process.

By boosting the inhibition of these factors, Fragmin helps keep the blood from clotting too much.

Fragmin has a minimal effect on other blood tests related to clotting, like the activated partial thromboplastin time (APTT), ensuring it mainly targets the clotting factors it is designed to affect.

Dosage Information and Usage

Dosage Form and Strengths of Fragmin

  • Injection: 2,500 IU/0.2 mL
  • Injection: 5,000 IU/0.2 mL
  • Injection: 7,500 IU/0.3 mL
  • Injection: 12,500 IU/0.5 mL
  • Injection: 15,000 IU/0.6 mL
  • Injection: 18,000 IU/0.72 mL
  • Injection: 10,000 IU/mL (graduated syringes)
  • Injection: 95,000 IU/3.8 mL (multiple-dose vials)
  • Injection: 10,000 IU/4.0 mL (single-dose vials)

Usage

Fragmin is used in different ways depending on the condition being treated.

For preventing complications in unstable angina or non-Q-wave myocardial infarction, the usual dose is 120 IU per kilogram of body weight, up to a maximum of 10,000 IU, given every 12 hours along with aspirin until the patient is stabilized, usually for 5 to 8 days.

For preventing deep vein thrombosis (DVT) after hip or abdominal surgery, doses vary based on the timing related to surgery and risk level, typically ranging from 2,500 IU to 5,000 IU once daily for 5 to 10 days.

In cancer patients with symptomatic venous thromboembolism (VTE), Fragmin is administered at 200 IU per kilogram of body weight once daily for the first month, then adjusted to about 150 IU per kilogram daily for the next five months.

For children with VTE, doses are adjusted based on age and monitored with anti-Xa levels to ensure effectiveness.

Storage Information

Store Fragmin at room temperature between 20°C and 25°C (68°F to 77°F).

It is also acceptable for the temperature to range from 15°C to 30°C (59°F to 86°F).

After the first use of the multiple-dose vials, keep them at room temperature for up to 2 weeks.

Do not use any Fragmin that has been stored outside these conditions or for longer than the specified time.

For the prefilled and graduated syringes, discard any unused portion after each use.

Be aware that the needle shield of the prefilled syringes may contain natural rubber latex, which can cause allergic reactions in some individuals.

Side Effects

Common Side Effects

  • Hematoma at injection site: Bruising or swelling where the injection was given.
  • Bleeding complications: May include minor bleeding from various sites.
  • Injection site pain: Discomfort or pain at the area where the drug is injected.
  • Elevations in liver enzymes: Increase in AST or ALT levels in blood tests.

Serious but Rare Side Effects

  • Major bleeding: Severe bleeding events, which may include:
    • Spinal/Epidural hematomas: Bleeding around the spinal cord or in the epidural space, which can lead to paralysis.
    • Intracranial hemorrhage: Bleeding within the brain.
    • Retroperitoneal hemorrhage: Bleeding behind the abdominal cavity.
  • Thrombocytopenia: Low platelet count, which can lead to increased bleeding risk.
  • Allergic reactions: Severe allergic reactions, including anaphylaxis.
  • Osteoporosis: Weakening of the bones with long-term use.
  • Skin necrosis: Severe skin damage or death in the area of injection.

Warnings and Precautions

  • Risk of Hemorrhage including Spinal/Epidural Hematomas: Hemorrhage or bleeding can occur with Fragmin when used alongside spinal or epidural anesthesia. The risk increases with procedures like spinal puncture or the use of indwelling epidural catheters. Patients should be monitored for bleeding symptoms and report any unusual back pain or leg weakness immediately.
  • Thrombocytopenia: Fragmin can cause a decrease in platelet count, known as thrombocytopenia. This condition may lead to severe complications such as clotting issues or, in extreme cases, amputation. Regular blood tests are necessary to keep track of platelet levels during treatment.
  • Risk of Serious Adverse Reactions in Neonates and Infants due to Benzyl Alcohol Preservative: Benzyl alcohol, used as a preservative in some Fragmin formulations, can cause serious reactions in infants, including “gasping syndrome.” This condition involves severe respiratory and metabolic issues. Use preservative-free Fragmin for infants to avoid these risks.
  • Laboratory Tests: Routine blood tests are required to monitor for side effects while on Fragmin. Standard tests like Prothrombin Time (PT) are not effective for monitoring Fragmin’s activity; anti-Xa levels are more appropriate. This is crucial for patients with kidney problems or abnormal bleeding.

Overdosage

Taking too much Fragmin can lead to serious bleeding complications.

To treat an overdose, a medication called protamine sulfate is used, which helps reverse the effects of Fragmin.

Typically, 1 mg of protamine is given for every 100 units of Fragmin administered. If bleeding continues, a second dose of protamine may be needed.

However, even with this treatment, the blood’s ability to clot may not fully return to normal.

It’s important to administer protamine sulfate carefully, as it can cause severe reactions like low blood pressure or allergic responses.

Always ensure emergency procedures are ready when using protamine sulfate.

Drug Interactions With Other Medications

Using Fragmin with other medications that affect blood clotting, such as oral anticoagulants, platelet inhibitors, and thrombolytic agents, can increase the risk of bleeding.

Oral anticoagulants are medicines taken by mouth to prevent blood clots.

Platelet inhibitors prevent blood cells called platelets from sticking together and forming clots. Thrombolytic agents help dissolve blood clots.

Combining Fragmin with these drugs makes it more likely that bleeding will occur.

It’s important to monitor for signs of bleeding and adjust medications as needed when using Fragmin with these other treatments.
