What is Jentadueto?
Jentadueto is a prescription medication that helps lower blood sugar in adults with type 2 diabetes. It contains two medicines, linagliptin and metformin HCl, used alongside diet and exercise.
Jentadueto is not suitable for people with type 1 diabetes or those with severe kidney problems, metabolic acidosis, or allergies to its ingredients. If you have a history of pancreatitis, kidney, liver, or heart problems, or if you are pregnant or breastfeeding, you should talk to your doctor before taking Jentadueto.
It is also important to tell your doctor if you drink alcohol frequently or are going to have an x-ray procedure with contrast dye.
How Does Jentadueto Work?
Jentadueto works by combining two medicines, linagliptin and metformin, to help control blood sugar levels in people with type 2 diabetes.
Linagliptin increases certain hormones that help your body produce more insulin when your blood sugar is high and reduces the amount of sugar your liver makes.
Metformin helps lower blood sugar by decreasing the amount of sugar your liver releases, reducing the absorption of sugar from your food, and improving how your body uses insulin.
Together, these medicines help keep your blood sugar levels more stable.

Usage
Jentadueto is used to improve blood sugar control in adults with type 2 diabetes. It should be taken twice daily with meals.
Your doctor will decide the right dose for you based on how well you respond to and tolerate the medication. If you have never taken metformin before, you will start with the lowest dose.
If you are already on metformin, your dose will be adjusted to include linagliptin. If you have kidney problems, your doctor will check your kidney function before and during treatment.
You should stop taking Jentadueto if you are having certain types of x-ray procedures and restart it only after your doctor confirms that your kidney function is stable.
Storage Information
Store Jentadueto at room temperature, between 68°F and 77°F (20°C to 25°C). It can handle short-term changes in temperature between 59°F and 86°F (15°C to 30°C). Keep the medication away from high humidity.

Warnings and Precautions
- Lactic Acidosis: Metformin can cause a rare but serious condition called lactic acidosis, which is a buildup of lactic acid in the blood. Symptoms include feeling weak, muscle pain, trouble breathing, stomach pain, and feeling sleepy.
- Renal Impairment: Jentadueto can be dangerous for patients with severe kidney problems. It is not recommended for those with an eGFR (a measure of kidney function) below 30 mL/min/1.73 m².
- Age 65 or Greater: Older adults are at higher risk for lactic acidosis due to potential kidney, liver, or heart issues. Their kidney function should be checked more often.
- Radiological Studies with Contrast: Imaging procedures using contrast dye can affect kidney function and increase the risk of lactic acidosis. Jentadueto should be stopped before such procedures and restarted only after confirming stable kidney function.
- Surgery and Other Procedures: During surgeries or procedures that limit food and fluid intake, Jentadueto should be temporarily discontinued to prevent dehydration and kidney problems.
- Hypoxic States: Conditions causing low oxygen levels, like heart failure or infections, can increase the risk of lactic acidosis. Jentadueto should be stopped if such conditions occur.
- Excessive Alcohol Intake: Drinking too much alcohol can increase the risk of lactic acidosis. Patients should avoid heavy alcohol use while taking Jentadueto.
- Hepatic Impairment: Patients with liver problems are at higher risk for lactic acidosis and should not use Jentadueto.
- Pancreatitis: Linagliptin, one of the components of Jentadueto, has been linked to inflammation of the pancreas. If symptoms like severe stomach pain occur, the medication should be stopped immediately.
- Hypoglycemia with Insulin Use: Combining Jentadueto with insulin or insulin-releasing drugs can cause low blood sugar. Doses of these medications may need to be adjusted.
- Hypersensitivity Reactions: Serious allergic reactions, including swelling, rash, and difficulty breathing, can occur. Jentadueto should be discontinued if these symptoms appear.
- Vitamin B12 Deficiency: Long-term use of metformin can lower vitamin B12 levels, leading to anemia. Vitamin B12 levels should be monitored periodically.
- Severe Joint Pain: Linagliptin may cause severe joint pain. If this occurs, stopping Jentadueto may be necessary.
- Bullous Pemphigoid: A skin condition causing blisters has been reported with linagliptin. Patients should stop Jentadueto if blisters or skin erosion develop and consult a doctor.
- Heart Failure: There is a potential risk of heart failure with DPP-4 inhibitors like linagliptin, especially in patients with previous heart or kidney problems. Symptoms such as shortness of breath and swelling should be monitored and reported immediately.
Overdosage of Jentadueto
If you take too much Jentadueto, call the Poison Help line at 1-800-222-1222 or seek medical help immediately.
An overdose of metformin in Jentadueto can lead to a serious condition called lactic acidosis, which affects about one-third of overdose cases.
Symptoms include muscle pain, trouble breathing, stomach pain, and feeling very weak or tired. Metformin can be removed from the body using a process called hemodialysis, which helps clear the drug from the bloodstream.
However, linagliptin, another component of Jentadueto, cannot be effectively removed through dialysis.
Drug Interactions With Other Medications
- Carbonic Anhydrase Inhibitors: Drugs like topiramate can cause changes in blood acidity, increasing the risk of lactic acidosis when taken with Jentadueto. Monitor patients more frequently if they are taking these medications together.
- Drugs that Reduce Metformin Clearance: Medications such as ranolazine and cimetidine can interfere with how metformin is eliminated from the body, raising the risk of lactic acidosis. Weigh the benefits and risks before using these drugs with Jentadueto.
- Alcohol: Alcohol can enhance the effect of metformin on lactate metabolism, leading to lactic acidosis. Advise patients to avoid excessive alcohol intake while on Jentadueto.
- Insulin or Insulin Secretagogues: Combining Jentadueto with insulin or sulfonylureas increases the risk of low blood sugar. Patients may need lower doses of these medications to prevent hypoglycemia.
- Drugs Affecting Glycemic Control: Certain drugs, such as diuretics and corticosteroids, can cause high blood sugar, reducing the effectiveness of Jentadueto. Closely monitor blood sugar levels when starting or stopping these drugs in patients taking Jentadueto.
- Inducers of P-glycoprotein or CYP3A4 Enzymes: Drugs like rifampin can decrease the effectiveness of linagliptin by lowering its levels in the body. Consider alternative treatments if taking Jentadueto with these enzyme inducers.
