What is Kerendia?
Kerendia (finerenone) is a prescription medication used to lower the risk of serious kidney and heart problems in adults with chronic kidney disease (CKD) linked to type 2 diabetes (T2D).
It helps reduce the chance of worsening kidney function, end-stage kidney disease (when kidneys stop working completely), heart-related deaths, heart attacks, and hospital visits for heart failure.
Kerendia is a tablet that contains the drug finerenone and is part of a class of medicines called nonsteroidal mineralocorticoid receptor antagonists (MRAs).
This works by blocking certain hormone effects that can harm the kidneys and heart. Kerendia was approved by the FDA in 2021.
It is not suitable for people taking strong CYP3A4 inhibitors (medicines that affect how other drugs are processed in the body) or those with adrenal insufficiency (a condition where the adrenal glands don’t produce enough hormones).
How Does Kerendia Work?
Kerendia works by blocking a specific receptor called the mineralocorticoid receptor (MR) that can become overactive in the kidneys, heart, and blood vessels.
When this receptor is overactive, it can cause problems like fibrosis (a buildup of scar tissue) and inflammation, which can harm these organs. Kerendia’s active ingredient, finerenone, stops this overactivity, helping to protect the kidneys and heart from damage.
Unlike some other medications for diabetes or high blood pressure, Kerendia specifically targets this receptor to reduce these risks.
Dosage Information and Usage
Dosage Form and Strengths of Kerendia
Kerendia is available in tablet form.
- 10 mg tablet: Pink, with “FI” on one side and “10” on the other.
- 20 mg tablet: Yellow, with “FI” on one side and “20” on the other.
Usage
Before starting Kerendia, your doctor will check your potassium levels and kidney function.
The starting dose of Kerendia depends on your kidney function:
- If your kidney function is good (eGFR ? 60), you will take 20 mg once a day;
- If your kidney function is moderate (eGFR 25-59), you will take 10 mg once a day.
- If your kidney function is low (eGFR < 25), Kerendia is not recommended
If you have difficulty swallowing the tablet, you can crush it and mix it with water or soft food like applesauce just before taking it. Your doctor will monitor your potassium levels and may adjust your dose based on these results.
If you miss a dose, take it as soon as you remember on the same day. If you miss it for the day, skip the dose and continue with your next dose as scheduled.
Side Effects
Common Side Effects
- Low blood pressure
- Low sodium levels in the blood
- High potassium levels in the blood (hyperkalemia)
- Dizziness or feeling faint
Serious but Rare Side Effects
- High potassium levels in the blood
- Allergic reactions, including rash, itching, or swelling
If you experience any serious side effects, contact your doctor immediately or call emergency services if necessary.
Warnings and Precautions
- Hyperkalemia: Kerendia can cause high potassium levels in the blood. This risk increases if you have poor kidney function or already high potassium levels. Your doctor will check your potassium levels and kidney function before starting Kerendia and may adjust the dose based on these results. Frequent monitoring might be needed for those at higher risk.
- Pregnancy: There is limited information on the safety of Kerendia during pregnancy. The general risk of birth defects and miscarriage in pregnancies is 2-4% and 15-20%, respectively.
- Lactation: There is no data on whether Kerendia passes into human breast milk or its effects on a breastfeeding baby. It’s best to avoid breastfeeding during treatment and for 1 day after stopping Kerendia.
- Pediatric Use: Kerendia has not been tested for safety and effectiveness in children under 18.
- Geriatric Use: Kerendia is used in older adults without needing dose adjustments. Studies show similar safety and effectiveness in patients aged 65 and older compared to younger adults.
- Hepatic Impairment: Do not use Kerendia if you have severe liver problems. For mild or moderate liver issues, no dose adjustment is needed, but your potassium levels should be monitored closely.
Overdosage of Kerendia
If you believe you’ve taken too much Kerendia, stop or reduce the dose immediately. An overdose can lead to high blood potassium levels (hyperkalemia).
If hyperkalemia happens, standard medical treatments should be used. Since finerenone in Kerendia mostly binds to blood proteins, dialysis isn’t effective for removal.
Seek medical help right away if an overdose is suspected.
Drug Interactions With Other Medications
- Strong CYP3A4 Inhibitors: Using Kerendia with strong CYP3A4 inhibitors, such as certain antifungal drugs or HIV medications, can increase the level of finerenone in your blood, raising the risk of side effects. It is advised to avoid combining Kerendia with these inhibitors and to steer clear of grapefruit or grapefruit juice.
- Moderate and Weak CYP3A4 Inhibitors: When Kerendia is taken with moderate or weak CYP3A4 inhibitors, like some antibiotics or blood pressure drugs, it may also elevate finerenone levels and the associated risk of side effects. Regular monitoring of potassium levels and possible dose adjustments may be required.
- Strong and Moderate CYP3A4 Inducers: Combining Kerendia with strong or moderate CYP3A4 inducers, such as certain medications used for seizures, can decrease the effectiveness of Kerendia by lowering finerenone levels. It is best to avoid using Kerendia with these inducers.
- Drugs That Affect Serum Potassium: If you are taking other drugs or supplements that increase potassium levels, more frequent monitoring of your potassium levels is necessary to prevent complications.

Here are the following alternative medications for Kerendia:
- Jardiance (empagliflozin)
