What is Kesimpta?
Kesimpta (ofatumumab) is a prescription medication used to treat adults with certain types of multiple sclerosis (MS).
MS is a disease where the immune system attacks the protective covering of nerve fibers, leading to problems with movement and other functions. Kesimpta is specifically used for:
- Clinically isolated syndrome: A first episode of MS symptoms.
- Relapsing-remitting disease: MS with periods of worsening symptoms followed by periods of improvement.
- Active secondary progressive disease: MS that starts with relapses and then gradually worsens.
Before starting treatment, tell your doctor if you have any infections, have taken immune system-affecting medicines, or if you’re planning any vaccinations.
Kesimpta may affect your immune system, therefore, certain vaccines need to be given before starting the medicine. If you’re pregnant or breastfeeding, discuss with your doctor whether Kesimpta is safe for you.
How Does Kesimpta Work?
Kesimpta works by targeting a specific protein called CD20 found on certain immune cells, particularly B cells, which are involved in multiple sclerosis (MS).
Although the exact way Kesimpta helps with MS isn’t fully understood, it is believed that the medicine binds to CD20 on B cells.
This binding leads to the destruction of these B cells through processes involving other immune system components.
By reducing the number of B cells, Kesimpta helps lessen the activity of MS and its symptoms.
After starting Kesimpta, B cell counts drop significantly, showing that the medicine is effectively targeting and reducing these cells in the body.
Dosage Information and Usage
Dosage Form and Strengths of Kesimpta
Kesimpta is available in the following forms and strengths:
- Injection: 20 mg/0.4 mL in a single-dose prefilled Sensoready Pen
- Injection: 20 mg/0.4 mL in a single-dose prefilled syringe
Usage
Kesimpta is administered through a subcutaneous injection, meaning it is injected under the skin.
You or a caregiver can give the injection in three areas: the thigh, the stomach area (abdomen), or the outer upper arm.
Your doctor will guide you on how to properly prepare and administer Kesimpta.
- Initial Dosing: The starting dosage is 20 mg of Kesimpta injected at Weeks 0, 1, and 2. There is no injection at Week 3.
- Ongoing Dosing: Beginning at Week 4 and continuing monthly, you will administer a 20 mg dose of Kesimpta.
- Missed Doses: If you miss a dose, take it as soon as you remember. After that, return to your regular monthly schedule.
- Injection Site: Avoid injecting into areas where the skin is tender, bruised, or has moles, scars, or stretch marks.
Preparation for Injection:
- Before Injection: Take the Kesimpta Sensoready Pen out of the refrigerator 15 to 30 minutes before use to reach room temperature. Check the liquid through the viewing window; it should be clear to slightly cloudy. Do not use it if it contains visible particles or is cloudy. Verify the expiration date and discard the pen if it has expired.
- Cleaning: Clean the chosen injection site with an alcohol wipe and let it dry completely before injecting.
During Injection:
- Cap Removal: Remove the cap from the Kesimpta Sensoready Pen only when ready to use it. Twist off the cap and discard it. Use the pen within 5 minutes of removing the cap.
- Injection Process: Hold the pen at a 90-degree angle to the cleaned skin. Press firmly to start the injection; you will hear two clicks. Keep the pen in place until the green indicator fills the window and stops moving.
After Injection:
- Site Care: You may see a small amount of blood at the injection site. Press a cotton ball or gauze over it for 10 seconds. You may use an adhesive bandage if needed.
- Disposal: Dispose of used Kesimpta Sensoready Pens in an FDA-cleared sharps disposal container. Do not throw them in household trash. For detailed disposal instructions, refer to the FDA’s website or local regulations.

Warnings and Precautions
- Infections: Serious infections, including bacterial, fungal, and viral, can occur during or after treatment with Kesimpta. In clinical studies, the infection rates for Kesimpta were similar to those for another MS drug, teriflunomide. Common infections with Kesimpta included upper respiratory and urinary tract infections. It’s important to delay Kesimpta administration if you have an active infection until it is resolved.
- Possible Increased Risk of Immunosuppressant Effects with Other Immunosuppressants: Combining Kesimpta with other immunosuppressive therapies might increase the risk of overall immune suppression. Kesimpta has not been studied in combination with other MS treatments, so this interaction is a potential concern.
- Hepatitis B Virus (HBV) Reactivation: Although no HBV reactivation has been reported with Kesimpta in MS patients, it has occurred with similar drugs used for other conditions. HBV can cause severe liver problems and even death. Patients should be tested for HBV before starting Kesimpta. Those who test positive or are carriers should be monitored closely by liver disease experts.
- Progressive Multifocal Leukoencephalopathy (PML): PML is a serious brain infection caused by the JC virus, which affects those with weakened immune systems. No cases were reported in Kesimpta MS studies, but PML has occurred with other anti-CD20 drugs. Symptoms of PML include sudden weakness, vision problems, and changes in thinking. If PML is suspected, Kesimpta should be stopped immediately.
- Vaccinations: Get all recommended vaccines at least 4 weeks before starting Kesimpta, especially live vaccines. Kesimpta may affect how well-inactivated vaccines work. Live or live-attenuated vaccines are not recommended during Kesimpta treatment or until B-cells are back to normal. Infants born to mothers who used Kesimpta should not receive live vaccines until their B-cell counts are normal.
- Injection-Related and Hypersensitivity Reactions: Kesimpta can cause serious reactions at the injection site or more severe allergic reactions. Common reactions include fever and headache, often within 24 hours of the first injection. Serious reactions such as anaphylaxis or difficulty breathing may occur.
- Reduction in Immunoglobulins: Kesimpta can lower levels of immunoglobulins, which are crucial for fighting infections. Decreased levels of immunoglobulin M (IgM) were observed more often with Kesimpta compared to another drug, teriflunomide.
- Fetal Risk: Kesimpta can harm the fetus by reducing B-cells and antibody responses in the baby. Pregnant women or those planning to become pregnant should use effective contraception while on Kesimpta and for at least 6 months after their last dose.
Drug Interactions With Other Medications
Using Kesimpta together with other immunosuppressive or immune-modulating therapies, such as systemic corticosteroids, may raise the risk of infections due to the combined effects on the immune system.
It is important to consider these additive immune effects when combining Kesimpta with other drugs that affect immune function.
When switching from other immune-modulating treatments to Kesimpta, consider the duration and effects of these prior therapies. They could add to the immunosuppression when beginning Kesimpta.
Alternative Medications
For individuals seeking alternatives to Kesimpta, the following medications may be considered:
- Ocrevus (ocrelizumab)

- Aubagio (teriflunomide)
- Copaxone (glatiramer)
- Tecfidera (dimethyl fumarate)
- Vumerity (diroximel fumarate)
