What are Levemir Penfill Cartridges?
Levemir Penfill Cartridges are pre-filled glass cartridges containing Levemir, a long-acting insulin used to manage blood sugar levels in people with diabetes.
These cartridges are designed to be used with Novo Nordisk insulin delivery devices like NovoPen.
Levemir helps control blood sugar levels and can be used alongside other diabetes medications or short-acting insulins. It’s only available with a doctor’s prescription and is not suitable for treating diabetic ketoacidosis.
Dosage Information and Usage
Dosage Form and Strengths of Levemir Penfill Cartridges
Levemir penfill cartridges contain “insulin detemir” at a concentration of 100 units/mL (U-100). These cartridges are designed for single-patient use only. It contains 3 mL of insulin and is specifically prescribed by a doctor for managing diabetes.
Usage
- Prepare for Injection:
-
- Wash your hands thoroughly.
- If the Penfill cartridge is already in your insulin delivery device, check the remaining insulin. If there’s not enough for your dose, follow the instructions in the device manual to replace it.
- Load a new Levemir Penfill cartridge into the device if necessary.
- Attach a new NovoFine needle to the device.
- Check for Insulin Flow (Priming):
-
- Prime your insulin delivery device to ensure insulin flow as described in the device user manual. This removes air bubbles and ensures the needle is functioning.
- Look for a drop of insulin at the needle tip before dialing up your required dose.
- Injecting a Dose:
-
- Choose an injection site on your abdomen, thigh, upper arm, or buttock. Rotate sites to avoid using the same spot too frequently.
- Pinch the skin at the injection site, insert the needle into the pinched skin, and inject the full dose of insulin.
- Keep the needle under your skin for at least 6 seconds, keeping the push button fully depressed until the needle is withdrawn.
- Apply gentle pressure to the injection site without rubbing it.
- After Injection:
-
- Remove and discard the needle in a sharps container after each injection to prevent contamination and ensure accurate dosing.
- Do not leave the needle attached to the insulin delivery device.
- Store the Penfill cartridge in the device until it needs to be replaced.
- Do not share needles, cartridges, or pens to avoid infections and ensure safety.
Storage Information
To maintain Levemir Penfill cartridges’ quality, store them in a fridge at 2-8°C before use. Once in use or as a spare, store at room temp (up to 30°C) for 4 weeks.
Discard after. Avoid freezing, heat, and light. Keep it in a carton, away from kids. Dispose of used needles safely. Ask the pharmacist for the disposal of expired medication.

Warnings and Precautions
- Never Share Insulin Devices: Do not share your Levemir FlexPen, needle, or insulin syringe with anyone, even if the needle is changed, to prevent the spread of blood-borne diseases like hepatitis or HIV.
- Changes in Insulin Regimen: Any change in your insulin routine, such as switching the type, strength, or injection site of your insulin, can impact your blood sugar levels. Always make these changes under medical supervision and monitor your blood sugar frequently to avoid complications like very low (hypoglycemia) or very high blood sugar (hyperglycemia).
- Hypoglycemia: Low blood sugar is a common side effect of Levemir and can be severe. Symptoms include dizziness, sweating, confusion, and shakiness. Factors like changes in diet, exercise, or medications can increase the risk of hypoglycemia. Severe hypoglycemia can cause seizures or unconsciousness and is potentially life-threatening.
- Medication Errors: To avoid accidentally using the wrong type of insulin, always check the insulin label before each injection.
- Hypersensitivity Reactions: Severe allergic reactions, including anaphylaxis (a life-threatening allergic reaction), can occur with Levemir. Symptoms include difficulty breathing, swelling of the face, tongue, or throat, and severe dizziness.
- Hypokalemia: Levemir can cause low potassium levels in your blood, which can lead to serious health issues like muscle weakness, irregular heartbeats (arrhythmias), or respiratory paralysis (inability to breathe). Monitor potassium levels if you are at risk, especially if you are taking medications that lower potassium levels.
- Fluid Retention and Heart Failure: Using Levemir with certain diabetes medications called PPAR-gamma agonists (e.g., thiazolidinediones) can cause your body to retain fluid, potentially leading to or worsening heart failure. Watch for symptoms like shortness of breath, swelling in your ankles or feet, and sudden weight gain.
Overdosage
If you take too much Levemir, your blood sugar may drop too low, causing hypoglycemia or a “hypo.” This can occur if you use more insulin than necessary, skip a meal, exercise excessively, or fall ill.
Signs of mild to moderate hypoglycemia include:
- sweating,
- pale skin,
- tiredness,
- nervousness,
- fast heartbeat,
- confusion,
- Hunger,
- headaches
Inform your family, friends, and colleagues about your diabetes and how to identify these symptoms.
If you faint, they should place you on your side and seek medical help without giving you anything to eat or drink to prevent choking. If you’re unconscious, they can administer a hormone called glucagon to help revive you.
Once awake, consume something sugary. If you still feel unwell, contact your doctor or go to the nearest hospital. If glucagon is ineffective, you may require additional treatment in a hospital. If you frequently experience hypoglycemia or have ever lost consciousness after using insulin, consult your doctor for guidance.
Drug Interactions With Other Medications
- Drugs That May Increase the Risk of Hypoglycemia: Certain medications, like antidiabetic agents (such as sulfonylureas), ACE inhibitors, and GLP-1 receptor agonists, can increase the risk of low blood sugar when taken with Levemir. This may cause your blood sugar to drop too low, leading to symptoms like sweating, shakiness, or confusion. If you’re on these medications, your doctor might reduce your Levemir dose and suggest checking your blood sugar levels more frequently to keep them in a safe range.
- Drugs That May Decrease the Blood Glucose Lowering Effect of Levemir: Some medications, such as corticosteroids and atypical antipsychotics (like olanzapine), can reduce the effectiveness of Levemir in controlling your blood sugar. As a result, your blood sugar might stay higher than desired. If you’re on these drugs, your healthcare provider may suggest increasing your insulin dose and checking your blood sugar levels more often to manage your diabetes effectively.
- Drugs That May Increase or Decrease the Blood Glucose Lowering Effect of Levemir: Medications like alcohol and beta-blockers can affect blood sugar levels when used with Levemir. Alcohol can first lower blood sugar, and then cause it to rise. Because of this unpredictability, you might need to adjust your insulin dose and monitor your blood sugar closely to avoid fluctuations.
- Drugs That May Blunt Signs and Symptoms of Hypoglycemia: Some drugs, like beta-blockers, can hide the warning signs of low blood sugar, such as a rapid heartbeat or anxiety. This makes it harder to notice when your blood sugar is dropping. If you’re on these medications, it’s crucial to check your blood sugar levels more often, as you might not notice symptoms until your blood sugar is dangerously low.
