What is Plegridy?

Plegridy (peginterferon beta-1a) is a prescription medication used to treat certain types of multiple sclerosis (MS) in adults.

MS is a disease where the immune system attacks the nervous system, causing problems with movement, balance, and coordination.

Plegridy helps manage relapsing forms of MS, including clinically isolated syndrome (first signs of MS), relapsing-remitting disease (periods of symptoms followed by recovery), and active secondary progressive disease (worsening over time).

It is not known if Plegridy is safe or effective for people under 18 or over 65. Do not use Plegridy if you are allergic to any of its ingredients, including interferon beta or peginterferon.

Before starting Plegridy, inform your doctor about any past or current medical conditions, especially the following:

  • mental health issues, liver problems,
  • low blood cell counts,
  • bleeding disorders,
  • heart issues, seizures,
  • thyroid problems,
  • autoimmune diseases

Tell your doctor if you are pregnant, planning to become pregnant, or breastfeeding, as Plegridy may affect these conditions.

Tell your clinician if you have a latex allergy because some Plegridy prefilled syringes contain a needle cap made from natural rubber (a latex derivative).

How Does Plegridy Work?

Plegridy helps manage multiple sclerosis (MS), but exactly how it works is not fully understood.

It is believed to help regulate the immune system, which is often overactive in MS.

This regulation may help reduce the number and severity of MS relapses.

After being injected, Plegridy reaches its highest level in the bloodstream within 1 to 1.5 days.

Its levels in the body increase with higher doses but do not build up significantly with regular use.

This means Plegridy maintains steady levels over time, potentially helping to manage MS symptoms effectively.

  • Pulmonary arterial hypertension (new or increasing fatigue, shortness of breath)
  • Autoimmune diseases (e.g., thyroid problems, autoimmune hepatitis)

Warnings and Precautions

  • Hepatic Injury: Plegridy may cause liver problems, including inflammation and severe liver damage. Monitor liver function regularly, as some patients have experienced elevated liver enzymes or liver injury. Discontinue Plegridy if significant liver issues occur.
  • Depression and Suicide: There is an increased risk of depression and suicidal thoughts with Plegridy. Patients should report any changes in mood or thoughts of self-harm immediately. If severe psychiatric symptoms develop, consider stopping Plegridy.
  • Anaphylaxis and Other Allergic Reactions: Rare but serious allergic reactions, such as anaphylaxis, can occur. Symptoms might include severe itching or swelling and trouble breathing. Stop Plegridy if a serious allergic reaction happens and seek medical attention.
  • Injection Site Reactions Including Necrosis: Some patients may experience severe reactions at the injection site, such as pain or necrosis. Ensure proper injection techniques and monitor for any severe reactions. Avoid injecting into areas with existing damage.
  • Congestive Heart Failure: Plegridy may worsen heart conditions in some patients. Regularly monitor patients with heart disease for any worsening symptoms. Seek medical advice if symptoms like shortness of breath or leg swelling occur.
  • Decreased Peripheral Blood Counts: Plegridy can reduce blood cell counts, which might lead to infections or bleeding. Regular blood tests are necessary to monitor for significant decreases. Patients with low blood counts should be closely monitored.
  • Thrombotic Microangiopathy (TMA): TMA is a serious condition involving blood vessel damage and can be life-threatening. Discontinue Plegridy if symptoms or lab results suggest TMA. Seek immediate medical treatment if TMA is suspected.
  • Pulmonary Arterial Hypertension (PAH): PAH, a serious lung condition, can develop during treatment. Watch for symptoms like new or worsening shortness of breath and fatigue. Discontinue Plegridy if PAH is diagnosed and managed accordingly.
  • Autoimmune Disorders: Plegridy may trigger autoimmune diseases affecting various organs. If new autoimmune symptoms appear, consider stopping Plegridy and evaluate for potential autoimmune disorders.
  • Seizures: Seizures have been reported with Plegridy use. Exercise caution if you have a history of seizures. Notify your healthcare provider if seizures occur or if you have concerns about seizure risks.

Drug Interactions With Other Medications

Plegridy may interact with other medications, which can affect how well it works or increase the risk of side effects.

When using Plegridy, inform your healthcare provider about all other drugs you are taking, including over-the-counter medicines and supplements.

This is important because combining Plegridy with certain medications might lead to more frequent or severe side effects, such as flu-like symptoms or injection site reactions.

Your doctor can help manage these interactions and adjust treatments as needed.

Frequently Asked Questions

Plegridy is a medication used to treat relapsing forms of multiple sclerosis (MS). It is an injectable drug that works by modifying the immune system to help reduce the frequency and severity of MS flare-ups.
To manage injection site reactions with Plegridy, use a new syringe and needle for each injection. Common reactions include redness, pain, or swelling at the injection site. If the site becomes swollen, painful, or appears infected and doesn’t heal within a few days, contact your healthcare provider immediately.
To manage flu-like symptoms with Plegridy, try these tips:
  • Use OTC medications: Ask your healthcare provider about using over-the-counter pain and fever reducers on your treatment days.
  • Stay hydrated: Drink plenty of water to stay well hydrated.
  • Adjust injection times: Experiment with different times of day for your injection, as symptoms may vary.
  • Keep a journal: Record the timing of side effects, water intake, and use of pain relievers.
Always consult your healthcare provider if you experience persistent or severe side effects.
Store Plegridy in the refrigerator between 36°F to 46°F (2°C to 8°C). If refrigeration is not available, you can store it at room temperature up to 77°F (25°C) for up to 30 days. Keep Plegridy in its original carton to protect it from light.
Do not take Plegridy if you are allergic to interferon beta, peginterferon, or any of the other ingredients in Plegridy. Also, avoid Plegridy if you have liver problems or worsening liver conditions. If you have a history of depression or suicidal thoughts, consult your healthcare provider before starting Plegridy.
Common side effects include flu-like symptoms such as fever, chills, and muscle aches. Injection site reactions like redness, pain, or swelling are also common. Serious but rare side effects include liver problems, severe allergic reactions, depression or suicidal thoughts, and heart issues.
Plegridy remains in your body for an extended period due to its pegylation, allowing it to stay effective over a longer duration. This extended action means that you do not need to take Plegridy as frequently as some other medications. The specific duration can vary depending on individual factors and treatment adherence.
Plegridy contains peginterferon beta-1a and belongs to the class of medications known as interferon betas. Interferons are proteins that occur naturally in the body and help reduce inflammation and decrease the immune response.
If you miss a dose of Plegridy, inject it as soon as you remember. After that, resume your regular dosing schedule, taking the next dose 14 days later as usual.
Plegridy should be injected once every 14 days (two weeks). It is administered under the skin (subcutaneously). For consistency, try to inject Plegridy at the same time on the same day each time.
Alcohol does not directly interact with Plegridy or affect how well it works. However, drinking alcohol can worsen some side effects of Plegridy, such as liver damage, a weakened immune system, and depression. It can also disrupt sleep, which may worsen MS symptoms. It is recommended to avoid alcohol while taking Plegridy.
Plegridy should be injected into your thigh, abdomen, or the back of your upper arm. Avoid injecting into areas where the skin is irritated, reddened, bruised, infected, or scarred. Use a different injection site each time to reduce the risk of irritation.
Plegridy is a long-acting interferon that helps manage multiple sclerosis (MS). Interferons are proteins naturally found in the immune system. Plegridy is believed to work by reducing inflammation and slowing the immune system’s attack on the protective covering of nerve fibers, called myelin.
Plegridy may be considered during pregnancy if clinically needed. Always consult with your doctor to discuss the potential risks and benefits before using Plegridy during pregnancy.
If you experience a serious allergic reaction to Plegridy, discontinue the medication immediately. The prefilled syringe contains natural rubber latex, which can cause allergic reactions in sensitive individuals.
Dispose of used Plegridy syringes or pens in a designated sharps disposal container. Do not throw them in your household trash. Follow local regulations for proper disposal of sharps containers.
Yes, Plegridy can cause liver problems. It may lead to liver damage, which could potentially result in symptoms such as weight gain. Regular liver function tests are recommended during treatment to monitor for any potential liver issues.

If you experience symptoms of depression or have thoughts of suicide while using Plegridy, contact your doctor immediately.
Plegridy functions similarly to an immunosuppressant by reducing immune system activity to help treat MS. However, it is less likely to increase the risk of infections compared to stronger immunosuppressants used for conditions like organ transplants.
The duration of side effects from Plegridy can vary. Common side effects, like injection-site reactions, such as redness, swelling, and pain, usually improve within a few days. Serious side effects, such as depression or seizures, require immediate medical attention.
