What is Premarin Cream?
Premarin Cream is prescribed for women to help treat the vaginal symptoms of menopause, like vaginal thinning, dryness, irritation, and pain during sex. These symptoms are caused because the body makes less estrogen.
Premarin Cream contains the active ingredient conjugated estrogens which belong to the drug class, estrogen derivatives.
These medications work by regulating the amount of estrogen in your body, which relieves vaginal and urinary menopausal symptoms.
Premarin Cream is not recommended:
- If you are allergic or have had an allergic reaction to Premarin Cream or any of its ingredients.
- For children under 18 years old.
- If you had or have breast or uterine cancer.
- If you have unusual bleeding.
- If you’ve had blood clots, liver problems, stroke, or heart attack.
- If you are pregnant.
What are the Indications for Premarin Cream?
Premarin Cream is approved to help treat moderate to severe vaginal symptoms caused by menopause, like burning urination, painful intercourse and itching, dryness, and burning in or around the vagina.
How is Premarin Cream used?
Storage
Premarin Cream should be stored at room temperature between 68-77°F (20-25°C) with excursions permitted between 59-86°F (15-30°C).
Keep it out of reach of children.
Instruction
Use Premarin Cream exactly as directed by your doctor.
Screw the nozzle end of the applicator onto the tube and squeeze the cream until it reaches the mark of your dosage. Unscrew the applicator, insert it into the vagina, and slowly release the cream into the vagina with the plunger.
Clean the applicator thoroughly with soap and water after use. Do not use boiled water or water that is too hot.
Do not change your dosage and use more than is prescribed. If you used more than the prescribed amount of cream, contact your doctor immediately or seek emergency medical care.
If you miss a dose, administer it as soon as you remember, unless it’s almost time for your next dose. Do not catch up by administering two doses at once. If you used more Premarin cream than prescribed, contact your doctor immediately or seek emergency medical care.
Dosage
Premarin Cream is available in a 30-gram tube with plastic applicators. Each gram of Premarin Cream contains 0.625 mg of conjugated estrogens.
The dosage is based on your condition and your response to the treatment.
For vaginal thinning not caused by menopause, the recommended dosage is 0.5 grams of cream into the vagina once a day for 3 weeks. Then take a week off and repeat the cycle if necessary.
For vaginal dryness and irritation caused by menopause, the recommended dosage is 0.5 grams of cream into the vagina twice a week.
Premarin Cream should be used regularly to get the most benefit out of it and for the shortest time possible.
SIDE EFFECTS
It’s important to understand that although Premarin Cream is FDA-approved, it comes with side effects that may or may not affect you.
Before using Premarin Cream, you should discuss possible side effects with your doctor or pharmacist.
Common Side Effects
Not all side effects require medical attention. As your body adjusts to Premarin Cream, side effects may go away. Tell your doctor if you experience the following symptoms, and they become severe or do not go away on their own:
- Headache
- Tender breasts
- Irregular vaginal bleeding/spotting
- Nausea/vomiting
- Stomach issues: abdominal cramping/bloating
- Water retention, usually in the legs, feet, or hands
- Vaginal yeast infections – discharge, burning, irritation, itching
- Hair loss
- Leg, joint, pelvic, or back pain
- Acne
- Dizziness
- Change in sexual interest
- Flushing
Serious Side Effects
Severe adverse reactions while using Premarin Cream may occur. Seek emergency medical care or call 911 if you experience any of the following symptoms:
- Severe allergic reaction: a rash over your whole body, sweating, extreme drowsiness, swelling of your face, tongue, or throat, difficulty breathing or swallowing, feeling faint, dizziness, rapid heartbeat, shortness of breath, or becoming confused.
- Vaginal problems, unusual bleeding, spotting, breakthrough bleeding, itching, irritation, odor, or discharge.
- Blood clots: heart attack, stroke, deep vein thrombosis, pulmonary embolism.
- Breast lumps
- Cancer (breast, womb, or ovarian):lumps, pain, swelling, or changes in the breasts, swollen lymph nodes, bloating, extreme tiredness, stomach-area pain.
- Other:mood changes, ongoing nausea/vomiting, severe stomach pain, jaundice, dark urine, increased thirst, unusual sweating, severe headache, confusion, vision changes, trouble breathing, coughing up blood, sudden dizziness/fainting.

WARNINGS & PRECAUTIONS
Before taking Premarin Cream, tell your doctor or pharmacist if you:
- Are allergic to conjugated estrogen or any of its ingredients.
- Have any liver problems.
- Have any abnormal vaginal bleeding.
- Take any other medications.
- Have or ever had breast, endometrial, cervical, vaginal, ovarian, or hormone-related cancer.
- Are taking any progestins, additional estrogens, or estrogen agonists.
- Have ever had any blood clotting or bleeding disorders.
- Have or had a stroke, heart attack, or any cardiovascular disorders.
- Have diabetes, fibroids, epilepsy/seizures, high cholesterol or triglyceride levels, thyroid disease, kidney problems, or gallbladder disease.
- Are having any major surgery or will be on bed rest.
- Are pregnant or planning to become pregnant.
- Are breastfeeding or planning to breastfeed.
Blood clotting disorders
Estrogens like Premarin Cream could increase your chances of developing blood clots in the lungs or legs. Your risk may also increase with age if you are obese, smoke, or have a family history of blood clots. If you have to undergo major surgery or require bed rest, you may have to stop the use of Premarin Cream for about 4 to 6 weeks before surgery. Speak to your doctor about your risk.
Gallbladder problems
If you have a history of gallbladder disease, Premarin Cream may affect your condition. Tell your doctor if you have any gallbladder problems or develop any symptoms while using Premarin Cream, like abdominal pain with/without fever and chills.
Heart disease/stroke
Premarin Cream could increase your risk of heart disease or stroke. Get immediate medical attention if you develop any of these symptoms: chest pain, tightness/pressure, nausea, sudden dizziness, headache, loss of speech, vision changes, weakness/numbness in your arms and legs.
Breast cancer
Premarin Cream can increase your risk of developing breast cancer. Do not use Premarin Cream if you have a history of breast cancer. If you have a family history of breast cancer, breast nodules, fibrocystic disease of the breast, or abnormal mammograms, your doctor must monitor you closely. Do not apply Premarin Cream to your breasts.
Cancer of the uterus
Estrogen replacement therapy may increase your chances of endometrium/uterus cancer. If you take progestin with estrogen, it reduces your risk to that of a woman who is not using estrogen. It is recommended that if you still have your uterus, you should take progestin when using estrogen. Speak to your doctor for advice.
Dementia
If you are over the age of 65, you may be at increased risk of developing dementia. Your doctor will monitor your memory and intellectual function closely while you are using Premarin Cream.
Water retention
The estrogen in Premarin Cream can cause water retention. Your doctor will monitor you if you suffer from epilepsy, asthma, kidney, or heart problems while using Premarin Cream.
Birth control
Premarin Cream can weaken the rubber/latex of condoms, diaphragms, and cervical caps. Speak to your doctor about effective birth control methods while using Premarin Cream.
Pregnant or planning to become pregnant
Premarin Cream must not be used if you are pregnant or planning to become pregnant.
Breastfeeding or planning to breastfeed
Speak to your doctor if you are breastfeeding or planning to breastfeed. It is not known if Premarin Cream passes through breast milk and is not recommended
INTERACTIONS & CONTRAINDICATIONS
Some medications may interact with Premarin Cream (conjugated estrogens) and cause side effects or make medications less effective.
Before you use Premarin Cream, tell your doctor or pharmacist if you take any medications, herbal supplements, or vitamins.
Premarin Cream should be used for the shortest duration possible and only as long as needed.
Do not use Premarin Cream to prevent heart disease/attacks, strokes, or dementia.
CYP3A4 Inhibitors
Taking CYP3A inhibitors with Premarin Cream can cause your body to break Premarin Cream down too quickly. This can increase the levels of Premarin Cream absorbed into your body and the risk of side effects. You should avoid taking these medications together unless directed otherwise by your doctor.
