What is Remeron?
Remeron (mirtazapine) is an FDA-approved prescription medication used to treat major depressive disorder (MDD) in adults.
It is not known if Remeron is safe or effective for children. Do not take Remeron if you use a Monoamine Oxidase Inhibitor (MAOI), have stopped taking an MAOI in the last 14 days, are on the antibiotic linezolid, or are allergic to mirtazapine.
Before taking Remeron, inform your healthcare provider if you have a history of depression, bipolar disorder, low white blood cell count, glaucoma, heart problems, seizures, high cholesterol, low sodium, kidney or liver problems, low blood pressure, or if you are pregnant or breastfeeding.
How Does Remeron Work?
Remeron works by affecting certain chemicals in the brain that are linked to mood. While the exact way it works isn’t fully understood, it’s believed to increase the levels of norepinephrine and serotonin, which help improve mood.
It does this by blocking certain receptors in the brain, specifically the ?2-adrenergic receptors and some serotonin receptors.
This action enhances the activity of these mood-related chemicals.
Further, Remeron blocks histamine and other receptors, which can lead to its calming effects and sometimes cause drowsiness.
Dosage Information and Usage
Dosage Form and Strengths of Remeron
Remeron is available as an oral tablet. The tablets come in different strengths: 15 mg, which are oval, scored, and yellow with “MSD” debossed on one side and “T3Z” on the other side of the score line.
Usage
The starting dose for Remeron is 15 mg once daily, taken at night before sleep. If needed, the dose can go up to a max of 45 mg per day.
Adjustments should be done every 1 to 2 weeks to assess the response. Before starting, check for bipolar disorder history.
Allow a 14-day gap after stopping MAOI antidepressants. Adjustments may be needed with other meds interacting with Remeron.

Warnings and Precautions
- Suicidal Thoughts and Behaviors in Adolescents and Young Adults: Antidepressants can increase the risk of suicidal thoughts and behaviors in people 24 years old and younger; monitor for these signs, especially during the first few months of treatment or when doses are changed.
- Agranulocytosis: Agranulocytosis is a serious condition with a low white blood cell count, making it hard to fight infections; symptoms include sore throat, fever, and signs of infection, requiring immediate medical attention and discontinuation of the medication.
- Serotonin Syndrome: This life-threatening condition is caused by too much serotonin in the brain; symptoms include agitation, hallucinations, rapid heart rate, dizziness, and muscle twitching, necessitating immediate medical help.
- Angle-Closure Glaucoma: The medication can cause pupil dilation, potentially triggering glaucoma in people with narrow eye angles; symptoms include eye pain, vision changes, and redness, requiring medical attention.
- QT Prolongation and Torsades de Pointes: This heart rhythm problem can lead to Torsades de Pointes and sudden death; use caution in people with heart problems or those taking other medications affecting heart rhythm.
- Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS): DRESS is a severe reaction involving a rash, fever, swollen glands, and organ inflammation; if suspected, stop the medication and seek medical help immediately.
- Increased Appetite and Weight Gain: The medication can significantly increase appetite and cause weight gain, as reported in 17% of patients during clinical studies.
- Somnolence: This medication can cause significant drowsiness, affecting daily activities and requiring caution when operating machinery or driving.
- Activation of Mania or Hypomania: The medication can trigger manic episodes in people with bipolar disorder; screening for bipolar disorder is recommended before starting treatment.
- Seizures: Use cautiously in people with a history of seizures, as rare cases have been reported during treatment.
- Elevated Cholesterol and Triglycerides: The medication can cause increases in cholesterol and triglycerides levels; regular monitoring of blood lipid levels is advised.
- Hyponatremia: This condition involves low sodium levels in the blood, leading to symptoms like headache and confusion; elderly patients and those on diuretics are at higher risk.
- Transaminase Elevations: The medication can cause increases in liver enzymes, indicating potential liver damage; monitoring liver function is recommended, especially in those with pre-existing liver conditions.
- Discontinuation Syndrome: Abruptly stopping the medication can cause withdrawal symptoms such as dizziness and anxiety; gradual dose reduction is recommended.
- Use in Patients with Concurrent Illness: Patients with recent heart problems or significant heart disease should use this medication with caution.
Overdosage of Remeron
Overdosing on Remeron, whether taken alone or with other medications, can cause serious health issues.
Symptoms of an overdose may include confusion, extreme drowsiness, memory problems, and a rapid heartbeat.
In severe cases, especially when mixed with other substances, overdose can be fatal and lead to heart rhythm problems such as QT prolongation and Torsades de Pointes.
There is no specific antidote for a Remeron overdose.
Drug Interactions With Other Medications
- Monoamine Oxidase Inhibitors (MAOIs): Using Remeron with MAOIs can increase the risk of serotonin syndrome, a potentially life-threatening condition. Remeron should not be taken with MAOIs.
- Other Serotonergic Drugs: Taking Remeron with other drugs that increase serotonin levels can lead to serotonin syndrome. Patients should be monitored for symptoms, especially when starting or increasing the dose.
- Strong CYP3A Inducers: Drugs that speed up the breakdown of Remeron can lower its levels in the blood. The dose of Remeron might need to be increased if these drugs are used together.
- Strong CYP3A Inhibitors: Drugs that slow down the breakdown of Remeron can raise its levels in the blood. The dose of Remeron might need to be decreased if these drugs are used together.
- Cimetidine: This drug can increase the levels of Remeron in the blood. The dose of Remeron might need to be adjusted when taken with cimetidine.
- Benzodiazepines and Alcohol: Using these substances with Remeron can worsen cognitive and motor skill impairment. It is best to avoid using them together.
- Drugs that Prolong QTc Interval: Using Remeron with drugs that affect heart rhythm can increase the risk of dangerous heart problems. Caution is advised.
- Warfarin: Remeron can affect blood thinning and increase the risk of bleeding. Monitoring of blood clotting levels (INR) is necessary when taken with warfarin.
