What is Serevent Diskus?
Serevent Diskus (salmeterol xinafoate) is a prescription inhaler used to help people with asthma, exercise-induced bronchospasm (EIB), and chronic obstructive pulmonary disease (COPD) breathe more easily.
It contains salmeterol, which is a type of long-acting bronchodilator (LABA).
Serevent Diskus doesn’t offer fast relief for sudden breathing issues, so it’s not a substitute for a rescue inhaler.
This medication is usually prescribed for adults and children aged 4 years and older.
However, it’s not recommended for people whose asthma is already well-controlled with other medications.
Additionally, if you have a severe allergy to milk proteins or any of the ingredients in Serevent Diskus, you should not use this medication.
How Does Serevent Diskus Work?
Serevent Diskus uses salmeterol, a long-acting bronchodilator, to target specific lung receptors called beta2-adrenoceptors.
These receptors are more responsive to salmeterol than other drugs, effectively relaxing the airway muscles.
This relaxation opens up the airways for easier breathing. Salmeterol also boosts cyclic AMP levels in lung cells, which helps prevent the release of substances triggering allergic reactions and inflammation, like histamines.
Overall, Serevent Diskus keeps airways open and lowers the risk of asthma attacks and breathing issues.
Dosage Information and Usage
Dosage Form and Strengths of Serevent Diskus
Serevent Diskus is available as an inhalation powder in a device containing a foil blister strip. Each blister contains 50 micrograms (mcg) of the active ingredient salmeterol. This medication is designed for oral inhalation only, meaning it is inhaled through the mouth directly into the lungs.
Usage
Before using Serevent Diskus, make sure you have been properly instructed by a healthcare provider on how to use it correctly.
If children are using the inhaler, they should do so under the supervision of an adult.
It’s important to follow the prescribed dosage exactly: for asthma and COPD, the usual dose is one inhalation twice daily, about 12 hours apart.
If you’re using it to prevent breathing difficulties caused by exercise, take one inhalation at least 30 minutes before exercising.
Do not use more than the prescribed amount or more frequently than directed.
Storage Information
Serevent Diskus should be stored at room temperature, between 68°F and 77°F (20°C and 25°C), and kept in a dry place away from direct heat or sunlight.
It’s important to keep the inhaler out of the reach of children.
The inhaler comes in a moisture-protective foil pouch and should remain inside this pouch until you are ready to use it for the first time.
Once the pouch is opened, the inhaler should be used within six weeks or discarded when the dose counter reads “0,” whichever comes first.
The inhaler is designed for single use and should not be taken apart or reused.

Warnings and Precautions
- Asthma-Related Death: Using Serevent Diskus alone for asthma can increase the risk of asthma-related death. It should only be used with an inhaled corticosteroid (ICS) to manage asthma safely.
- Pediatric and Adolescent Patients: Children and teenagers using Serevent Diskus are at risk of asthma-related hospitalizations. It’s safer to use a combination inhaler that includes both a LABA (like Serevent) and an ICS for these age groups.
- Deterioration of Disease and Acute Episodes: Serevent Diskus should not be used during severe asthma or COPD flare-ups. It is not intended for immediate relief of acute symptoms. Use a rescue inhaler for sudden breathing problems.
- Not a Substitute for Corticosteroids: Serevent Diskus does not have the anti-inflammatory effects of corticosteroids. Continue taking your prescribed corticosteroids even if you feel better with Serevent Diskus.
- Excessive Use and Other LABAs: Do not use Serevent Diskus more often than prescribed or with other LABA medicines. Overuse can lead to serious side effects, including heart problems.
- Paradoxical Bronchospasm: Inhalation of Serevent Diskus may cause a sudden worsening of breathing. If this happens, use a rescue inhaler and stop using Serevent Diskus immediately.
- Cardiovascular and Central Nervous System Effects: Overuse of Serevent Diskus can lead to heart issues or nervous system effects like tremors or headaches. Use it cautiously if you have heart problems.
- Immediate Hypersensitivity Reactions: Severe allergic reactions, including anaphylaxis, may occur. People with severe milk protein allergies should avoid Serevent Diskus.
- Drug Interactions with Strong CYP3A4 Inhibitors: Avoid using Serevent Diskus with strong CYP3A4 inhibitors (like certain antifungals and antibiotics) to prevent increased cardiovascular side effects.
- Coexisting Conditions: Use Serevent Diskus carefully if you have conditions like seizures or thyroid problems, as it may worsen these issues.
- Hypokalemia and Hyperglycemia: Serevent Diskus can cause low potassium levels and high blood sugar in some patients. These effects are usually minor but should be monitored.
Overdosage
Taking too much Serevent Diskus can cause symptoms like:
- seizures,
- chest pain,
- high or low blood pressure,
- rapid or irregular heartbeats,
- nervousness,
- headaches,
- tremors,
- muscle cramps,
- dry mouth,
- palpitations,
- nausea,
- dizziness,
- Fatigue,
- trouble sleeping
It can also lead to dangerous heart rhythm changes and, in severe cases, cardiac arrest or death.
If an overdose occurs, stop using Serevent Diskus and seek medical help immediately.
Treatment includes stopping the medication, supportive care, and monitoring the heart.
A selective beta-blocker might be used carefully, but it can cause bronchospasm. Dialysis is not known to be helpful for overdose.
Drug Interactions With Other Medications
- Inhibitors of Cytochrome P450 3A4: Using Serevent Diskus with strong CYP3A4 inhibitors (like ritonavir or ketoconazole) is not recommended. These drugs can increase the amount of Serevent Diskus in your system, which may lead to more side effects, such as heart problems.
- Monoamine Oxidase Inhibitors and Tricyclic Antidepressants: Be very careful if you are taking monoamine oxidase inhibitors or tricyclic antidepressants, or if you have stopped them within the last two weeks. These drugs can enhance the effects of Serevent Diskus, possibly increasing its impact on the blood vessels.
- Beta-adrenergic Receptor Blocking Agents: Beta-blockers can reduce the effectiveness of Serevent Diskus and might cause severe breathing problems. They are generally avoided in people with asthma or COPD, but if needed, only cardioselective beta-blockers should be used with caution.
- Non–Non-Potassium-Sparing Diuretics: Taking Serevent Diskus with non–non-potassium-sparing diuretics (like loop or thiazide diuretics) can worsen changes in heart rhythm and lower potassium levels. Be cautious with this combination, especially if the dose of Serevent Diskus is higher than recommended.
