What is Synjardy?

Synjardy is a prescription medication that treats type 2 diabetes, combining two diabetes medicines: empagliflozin (Jardiance) and metformin HCl. It’s used alongside diet and exercise to help improve blood sugar levels in adults and children aged 10 years and older who have this condition.

Empagliflozin, one of the medicines in Synjardy, can also be used in adults with type 2 diabetes who have cardiovascular disease to lower the risk of cardiovascular death, and in those with heart failure to reduce the risk of cardiovascular death and hospitalization.

Synjardy is not meant for people with type 1 diabetes or those with certain kidney or metabolic conditions.

Dosage Information and Usage

Dosage Form and Strengths of Synjardy

Synjardy is available in tablet form. Here are the following dosages and strengths:

  • 5 mg empagliflozin / 500 mg metformin HCl – orange yellow, oval, film-coated tablet
  • 5 mg empagliflozin / 1000 mg metformin HCl – brownish yellow, oval, film-coated tablet
  • 12.5 mg empagliflozin / 500 mg metformin HCl – pale brownish purple, oval, film-coated tablet
  • 12.5 mg empagliflozin / 1000 mg metformin HCl – dark brownish purple, oval, film-coated tablet

Usage

Take Synjardy orally twice daily with meals. Swallow each tablet whole; do not split, crush, dissolve, or chew. Adjust dosage based on the patient’s current treatment regimen and response. Monitor and adjust dosage gradually to manage gastrointestinal side effects with metformin.

Warnings and Precautions

  • Lactic Acidosis: Metformin, a component of Synjardy, can rarely cause lactic acidosis, a serious condition where there is too much acid in the blood. Symptoms include muscle pain, stomach discomfort, and breathing problems. Lactic acidosis is a rare but severe side effect of metformin, which can cause muscle pain, stomach issues, and breathing difficulties due to increased acidity in the blood.
  • Renal Impairment: Synjardy is not recommended in patients with severe kidney impairment (eGFR less than 30 mL/min/1.73 m2) because metformin can build up in the body, increasing the risk of lactic acidosis. In severe kidney disease, the body cannot effectively clear metformin, leading to its accumulation and a higher risk of lactic acidosis, a serious metabolic condition.
  • Drug Interactions: Certain medications can increase the risk of lactic acidosis when taken with Synjardy, especially those that affect kidney function or acid-base balance. Drugs that affect how kidneys work or the balance of acids and bases in the body can increase the risk of lactic acidosis when combined with Synjardy.
  • Age 65 or Greater: Elderly patients are more prone to lactic acidosis due to age-related declines in organ function, particularly kidney function. Older adults are at higher risk of lactic acidosis because ageing kidneys may not clear metformin effectively, leading to its accumulation in the body.
  • Radiological Studies with Contrast: Stop Synjardy before imaging procedures using iodinated contrast agents to prevent kidney damage and lactic acidosis in patients with reduced kidney function. Imaging dyes can harm kidneys and worsen lactic acidosis risk in patients with poor kidney function. Stopping Synjardy before such procedures reduces this risk.
  • Surgery and Other Procedures: Temporarily discontinue Synjardy during surgeries or when food and fluid intake are restricted to prevent dehydration and kidney problems. Surgery or procedures with limited food and fluid intake can cause dehydration and kidney issues, so stopping SYNJARDY temporarily helps manage these risks.
  • Hypoxic States: Conditions like heart failure or severe infections can increase the risk of lactic acidosis with Synjardy, especially if there’s poor oxygen delivery to tissues. Heart failure or severe infections can reduce oxygen delivery to tissues, increasing the risk of lactic acidosis with Synjardydue to poor tissue oxygenation.
  • Excessive Alcohol Intake: Drinking too much alcohol while taking SYNJARDY can worsen the risk of lactic acidosis. Alcohol consumption while on SYNJARDY can amplify the risk of lactic acidosis by affecting how the body processes metformin, leading to increased acidity in the blood.
  • Hepatic Impairment: Synjardy should not be used in patients with liver disease because impaired liver function can lead to higher blood levels of metformin and increase the risk of lactic acidosis. Liver disease can reduce the body’s ability to clear metformin, causing it to accumulate and increase the risk of lactic acidosis, a serious condition.
  • Diabetic Ketoacidosis in Type 1 Diabetes: Synjardy is not approved for treating type 1 diabetes and can increase the risk of diabetic ketoacidosis (DKA), a dangerous condition with high acid levels in the blood. Synjardy is not meant for type 1 diabetes and can cause diabetic ketoacidosis, which is a severe condition where the body produces high levels of acids when it doesn’t get enough insulin.

Overdosage

Synjardy is a medication used to treat type 2 diabetes. It combines two active ingredients: empagliflozin and metformin hydrochloride. Empagliflozin helps lower blood sugar by causing the kidneys to remove sugar from the body through urine.

Metformin reduces the amount of sugar produced by the liver and helping the body respond better to insulin. Together, they help control blood sugar levels in adults with type 2 diabetes.

Drug Interactions

  • Lactic Acidosis: Metformin in Synjardy can rarely cause lactic acidosis, characterized by symptoms like muscle pain or abdominal discomfort. Prompt medical attention is crucial if suspected.
  • Diabetic Ketoacidosis in Type 1 Diabetes: Using Synjardy in type 1 diabetes can increase the risk of ketoacidosis, a serious condition with symptoms like nausea or breathlessness. Immediate medical help is needed if symptoms occur.
  • Volume Depletion: Empagliflozin can lead to dehydration or low blood pressure, especially in elderly patients or those with kidney issues. Doctors should assess fluid levels before starting treatment.
  • Urosepsis and Pyelonephritis: Some patients may develop serious urinary infections while on Synjardy, needing prompt treatment if symptoms like fever or urinary problems arise.
  • Hypoglycemia: Combining Synjardy with insulin or sulfonylureas can lower blood sugar too much, causing symptoms like dizziness or confusion. Close monitoring is necessary.
  • Necrotizing Fasciitis (Fournier’s Gangrene): Rarely, Synjardy may lead to severe infections in the genital area, requiring immediate medical attention if symptoms such as swelling or fever occur.
  • Genital Mycotic Infections: Empagliflozin increases the risk of fungal infections in the genital area, which should be monitored and treated promptly if symptoms develop.
  • Lower Limb Amputation: In some studies, there was an increased risk of lower limb amputations with SGLT2 inhibitors like empagliflozin. Patients should be advised on foot care and monitored for signs of infection.
  • Hypersensitivity Reactions: Rarely, serious allergic reactions may occur with Synjardy, requiring immediate discontinuation and medical evaluation if symptoms like swelling or difficulty breathing occur.
  • Vitamin B12 Deficiency: Long-term use of metformin in Synjardy may lower vitamin B12 levels, which should be monitored and supplemented if necessary every few years.

Frequently Asked Questions

Synjardy is a medication used to treat Type 2 diabetes. It combines two drugs, empagliflozin and metformin, to help lower blood sugar levels in adults with this condition.
Synjardy works in the body by combining two medications. Empagliflozin helps the kidneys remove excess sugar through urine, and metformin reduces the amount of sugar your liver makes and improves how your body uses insulin. Together, they help lower blood sugar levels in people with Type 2 diabetes.
You should not take Synjardy if you:
  • Have severe kidney problems, end-stage renal disease, or are on dialysis.
  • Have a condition called metabolic acidosis or diabetic ketoacidosis, which means increased ketones in the blood or urine.
  • Are allergic to empagliflozin (Jardiance), metformin HCL, or any of the ingredients in Synjardy. Signs of an allergic reaction include rash, hives, or swelling of the face, lips, mouth, or throat that may cause difficulty in breathing or swallowing. If you experience these symptoms, stop taking Synjardy and seek medical help immediately.
After you start taking Synjardy, you may experience lower blood sugar levels because it helps your body get rid of extra sugar through your urine. You might also notice an increase in how often you need to urinate. Additionally, your blood pressure may decrease slightly because Synjardy also helps your body get rid of extra salt.
Synjardy is a pill you take twice a day with food to help avoid an upset stomach. It comes in four different doses, and your doctor will choose the best one for you. It fits into your treatment plan by helping to lower your blood sugar levels. Make sure to take Synjardy exactly as your doctor tells you.
Yes, you can take Synjardy with other Type 2 diabetes medications, but you should ask your doctor first. Synjardy can be taken with medicines like sulfonylurea. However, taking Synjardy with other medications that lower blood sugar, like sulfonylurea or insulin, can increase your risk of low blood sugar.
Once you stop taking Synjardy, it can take up to 4 days for most of the medication to leave your body. This is based on how long it takes for the drug to be broken down and removed from your system.
  • Low blood sugar (hypoglycemia)
  • Urinary tract infections
  • Increased urination
  • Yeast infections
  • Nausea
  • Diarrhea
  • Stomach pain
  • Headache
  • Rash
Stop taking Synjardy and get medical help immediately if you have symptoms of lactic acidosis. These symptoms include:
  • Unusual tiredness
  • Dizziness
  • Severe drowsiness
  • Chills
  • Blue or cold skin
  • Muscle pain
  • Fast or difficult breathing
  • Slow or irregular heartbeat
  • Stomach pain with nausea, vomiting, or diarrhea
Yes, Synjardy can cause weight loss. In studies, people with type 2 diabetes who took Synjardy lost an average of 2-3% of their body weight. Take note that this is not FDA-approved medication for weight loss.
Here are some common alternative options to Synjardy:
  • Jardiance (empagliflozin)
  • Metformin
  • Glyxambi (empagliflozin/linagliptin)
  • Trijardy XR (empagliflozin/linagliptin/metformin ER)
  • Janumet (sitagliptin/metformin)
  • Farxiga (dapagliflozin)
  • Invokana (canagliflozin)
  • Januvia (sitagliptin)
  • Trulicity (dulaglutide)
  • Ozempic (semaglutide)
Synjardy may raise ketoacidosis risk in those with prior pancreatitis or pancreatic surgery, involving pancreas inflammation. Notify your doctor if you’ve had these conditions before starting Synjardy. They heighten ketoacidosis risk, which increases blood ketone levels and acidity.
Synjardy can lower your blood pressure, particularly if you become dehydrated. This may cause dizziness or lightheadedness when you stand up suddenly from sitting or lying down.
Synjardy should be taken twice a day with breakfast and dinner. It’s not recommended to take it at night.
Synjardy is currently available only as a brand-name medication. There is no generic version available. Generic drugs are exact copies of the active ingredients in brand-name medications. Synjardy contains two active drugs: empagliflozin and metformin.
The ideal times to take Synjardy are with breakfast as prescribed by your healthcare provider. This helps to minimize the risk of an upset stomach.
You should take Synjardy twice daily with meals. For Synjardy XR, take it once daily with a meal in the morning. Remember to swallow each tablet whole.
Boehringer Ingelheim Pharmaceuticals, Inc. is the manufacturer of Synjardy.
If you experience an allergic reaction to Synjardy, such as itchy skin, rash with blisters, or sores, stop taking it immediately and seek medical attention.
If you take too much Synjardy, you may experience severe symptoms such as intense drowsiness, severe nausea, vomiting, diarrhea, rapid breathing, or irregular heartbeat.
