What is Verquvo?

Verquvo (vericiguat) is a prescription medication for adults with chronic heart failure.

It is used to lower the risk of death and the need for hospitalization in people who have recently been in the hospital for heart failure or needed IV (intravenous) medicines, which are given through a vein.

Verquvo is for patients whose heart pumps less than 45% of the blood it holds with each beat. This is known as “ejection fraction,” a measure of how well the heart is pumping.

It is not known if Verquvo is safe for children. Do not take Verquvo if you are pregnant or taking another medicine called a soluble guanylate cyclase (sGC) stimulator, which is a type of drug that helps blood vessels relax and widen.

How Does Verquvo Work?

Verquvo works by helping your heart and blood vessels function better. It contains vericiguat, which stimulates an enzyme called soluble guanylate cyclase (sGC).

This enzyme is important because it helps produce a molecule called cyclic guanosine monophosphate (cGMP) when it interacts with nitric oxide (NO).

cGMP helps relax blood vessels and improve heart function. In people with heart failure, the body produces less NO and sGC doesn’t work well.

Verquvo boosts the activity of sGC, increasing cGMP levels, which leads to relaxed blood vessels and better blood flow.

Dosage Information and Usage

Dosage Form and Strengths of Verquvo

  • 2.5 mg tablets: Round, biconvex, white, film-coated, with “2.5” on one side and “VC” on the other side.
  • 5 mg tablets: Round, biconvex, brown-red, film-coated, with “5” on one side and “VC” on the other side.
  • 10 mg tablets: Round, biconvex, yellow-orange, film-coated, with “10” on one side and “VC” on the other side.

Usage

Verquvo is used to reduce the risk of cardiovascular death and the need for hospitalization in adults with chronic heart failure who have symptoms and have been recently hospitalized or needed IV diuretics.

The recommended starting dose is 2.5 mg taken orally once daily with food. The dose should be doubled approximately every 2 weeks until the target dose of 10 mg once daily is reached, based on how well the patient tolerates it.

If you can’t swallow the tablet whole, you can crush it and mix it with water before taking it.

Side Effects

Common Side Effects

  • Low blood pressure (hypotension)
  • Low red blood cells (anemia)

Serious but Rare Side Effects

  • Severe allergic reactions (difficulty breathing, swelling of face, lips, tongue, or throat)
  • Extreme dizziness or fainting
  • Unusual bleeding or bruising
  • Signs of infection (fever, chills, sore throat)

Warnings and Precautions

  • Pregnancy and Embryo-Fetal Toxicity: Verquvo may cause harm to an unborn baby based on animal studies. Women who can become pregnant should have a pregnancy test before starting treatment. They should use effective birth control during treatment and for at least one month after the last dose to prevent pregnancy. There is a Pregnancy Surveillance Program to monitor outcomes in women exposed to Verquvo during pregnancy. It is not known if Verquvo is safe during pregnancy.
  • Lactation: It is not known if Verquvo passes into human milk, but it is present in the milk of lactating rats. Due to potential risks to the baby, women should not breastfeed during treatment with Verquvo.
  • Pediatric Use: The safety and effectiveness of Verquvo have not been established in children.
  • Geriatric Use: No dosage adjustment is needed for elderly patients. In studies, older patients (65 years and older) did not show different safety or effectiveness compared to younger patients, but some older individuals may be more sensitive to the drug.
  • Renal Impairment: No dosage adjustment is needed for patients with mild to moderate kidney problems. Verquvo has not been studied in patients with severe kidney problems or those on dialysis.
  • Hepatic Impairment: No dosage adjustment is needed for patients with mild to moderate liver problems. Verquvo has not been studied in patients with severe liver problems.

Overdosage of Verquvo

Limited data exists on Verquvo overdosing in humans.

Studies tested doses up to 10 mg in heart failure patients and up to 15 mg in those with normal heart pumping (ejection fraction of 45%+). These higher doses were generally well tolerated.

Overdose symptoms may include low blood pressure (hypotension). Treatment aims to alleviate symptoms. Verquvo is unlikely to be eliminated by dialysis due to its strong protein binding in the blood.

Drug Interactions With Other Medications

  • Other Soluble Guanylate Cyclase Stimulators: Verquvo should not be used with other sGC stimulators. Combining these drugs can increase the risk of side effects, including low blood pressure.
  • PDE-5 Inhibitors: It is not recommended to use Verquvo with PDE-5 inhibitors, such as those used for erectile dysfunction or pulmonary hypertension. This combination can lead to a dangerous drop in blood pressure.

Frequently Asked Questions

Verquvo is used to reduce the risk of cardiovascular death and hospitalization due to heart failure in adults.
The possible side effects of Verquvo include low blood pressure (hypotension) and low red blood cells (anemia). Low blood pressure can cause dizziness or fainting, while anemia can lead to feelings of fatigue and weakness.
Verquvo works by helping the heart improve blood flow. It relaxes and widens the blood vessels, which makes it easier for the heart to pump more blood and oxygen throughout the body. This can help reduce symptoms and improve heart function in people with heart failure.
Verquvo is used to reduce the risk of cardiovascular death and hospitalization due to heart failure in adults. It is specifically indicated for individuals with symptomatic chronic heart failure who have an ejection fraction of less than 45% and who have recently been hospitalized for heart failure or needed outpatient IV diuretics.
Verquvo should not be used with other soluble guanylate cyclase (sGC) stimulators, as this can lead to an increased risk of side effects. Additionally, combining Verquvo with PDE-5 inhibitors, which are used for conditions like erectile dysfunction, is not recommended because it may cause a dangerous drop in blood pressure.
The recommended starting dose of Verquvo is 2.5 mg taken orally once daily with food. The dose can be doubled approximately every 2 weeks, depending on how well the patient tolerates it until the target maintenance dose of 10 mg once daily is reached.

You should discuss all the medicines you are taking with your doctor before starting Verquvo. Do not use Verquvo if you are already taking another soluble guanylate cyclase (sGC) stimulator.

If your doctor prescribes Verquvo and you are already taking Entresto, it is generally safe to take them together. However, your doctor may monitor your blood pressure closely, as both medications can cause low blood pressure.
Yes, Verquvo can lower blood pressure. One of the most common side effects of Verquvo is low blood pressure.
No, Verquvo is not a diuretic. It works by improving blood flow and relaxing blood vessels, but it does not help remove excess fluid from the body like diuretics do.

Alternatives to Verquvo include sacubitril/valsartan (Entresto), ACE inhibitors like benazepril (Lotensin) and lisinopril (Qbrelis), and ARBs such as candesartan (Atacand) and losartan (Cozaar).
No, Verquvo is not a blood thinner or a beta-blocker. It is a soluble guanylate cyclase stimulator, used to reduce certain risks of heart failure with reduced ejection fraction in adults.
