What is Zenhale?
Zenhale (mometasone furoate, formoterol fumarate dihydrate) is a medication prescribed to help control asthma in people aged 12 years and older.
It combines two medicines: mometasone furoate, an inhaled corticosteroid that reduces inflammation in the lungs, and formoterol fumarate dihydrate, a long-acting beta2-adrenergic agonist (LABA) that keeps the airways open.
Mometasone furoate decreases swelling and irritation in the airways to prevent asthma attacks, while formoterol fumarate dihydrate relaxes the muscles around the airways to improve airflow and make breathing easier for up to 12 hours.
Zenhale is used twice daily to manage chronic asthma that isn’t well controlled by other treatments.
However, Zenhale is not suitable for everyone. It should not be used if you are allergic to any of its ingredients, for sudden asthma attacks, or if you have untreated infections or certain heart conditions, such as tachyarrhythmia. It is also not intended for children under 12 years old.
How Does Zenhale Work?
Zenhale works by combining two medicines that help control asthma. Mometasone furoate is a type of steroid that reduces inflammation in the lungs.
It works by blocking the action of certain substances in the body that cause swelling and irritation in the airways.
This helps to keep the airways open and reduces asthma symptoms over time. Formoterol fumarate dihydrate is a bronchodilator that relaxes the muscles around the airways, making it easier to breathe.
It starts working quickly and helps keep the airways open for up to 12 hours. These two medicines help manage asthma symptoms and improve breathing.
Dosage Information and Usage

Form: Oral inhalation aerosol
Strengths:
- 50 mcg mometasone furoate & 5 mcg formoterol fumarate per puff
- 100 mcg mometasone furoate & 5 mcg formoterol fumarate per puff
- 200 mcg mometasone furoate & 5 mcg formoterol fumarate per puff
Usage
Prime the Inhaler:
- If using it for the first time, or if it hasn’t been used for over 5 days, remove the cap and shake the inhaler well.
- Release 4 puffs into the air, away from your face, to prime it.
Prepare for Use:
- Remove the cap from the mouthpiece.
- Check that the mouthpiece is clean and the canister is properly inserted.
Take Your Dose:
- Hold the inhaler upright and shake it.
- Breathe out fully through your mouth.
- Place the mouthpiece in your mouth and close your lips around it.
- While breathing in deeply and slowly, press down on the canister until it stops moving.
- Remove the inhaler from your mouth and hold your breath for up to 10 seconds, then breathe out through your nose.
Second Puff:
- Wait about 30 seconds.
- Shake the inhaler again and repeat steps 2 and 3.
After Use:
- Replace the cap firmly.
- Rinse your mouth with water and spit out the water. Do not swallow.
Check Dose Counter:
- The inhaler has a dose counter showing the number of puffs left.
- When the counter shows 20, refill your prescription or consult your doctor.
- Discard the inhaler after the counter reaches 0. Even if it feels like it still has some medication, it may not deliver the right amount.
Storage Information
Keep your inhaler in a safe place, out of reach, and sight of children to prevent any accidental harm.
Store your inhaler between 15°C and 30°C. If it gets too cold, warm it in your hand before using it.
For the inhaler with 120 puffs, you can store it in any position. For the inhaler with 60 puffs, after priming, store it with the mouthpiece facing down or sideways.

Warnings and Precautions
- Not for Acute Use: Zenhale should not be used to treat sudden asthma symptoms. Patients need to learn how to use other medications for acute asthma and have follow-ups to ensure proper usage.
- Discontinuance: Inhaled corticosteroids should not be stopped suddenly. The dose should be reduced gradually under a doctor’s supervision to avoid worsening asthma.
- Transferring from Systemic Corticosteroids: Patients switching from oral corticosteroids to Zenhale may need extra care. They should carry oral corticosteroids and a warning card for emergencies, and their adrenal function should be monitored.
- Systemic Effects of Corticosteroids: High doses of Zenhale can cause side effects such as Cushing’s syndrome, reduced bone density, and eye problems. Use the lowest effective dose to minimize these risks.
- Serious Asthma-Related Events: Using LABA alone for asthma can increase the risk of severe events like hospitalization or death. Combining LABA with inhaled corticosteroids does not significantly increase this risk compared to using inhaled corticosteroids alone.
- Other Long-Acting Beta2-Agonists: Do not use Zenhale with other long-acting beta2-agonists.
- Exceeding Recommended Dose: Do not exceed the recommended dose. Higher doses do not improve effectiveness and can increase side effects.
- Cardiovascular: Zenhale may slightly increase the QTc interval. Use with caution if you have heart problems or other conditions that affect heart rhythm.
- Ear/Nose/Throat: Oral fungal infections may occur. Rinse your mouth after using Zenhale and seek treatment if needed.
- Adrenal Suppression: High doses of inhaled corticosteroids may affect adrenal gland function. Additional systemic corticosteroids might be needed during stress or surgery.
- Bone Metabolism: Long-term use can lead to bone loss. Monitor bone health, especially if using high doses or if you are post-menopausal.
- Hypokalemia and Hyperglycemia: Zenhale can cause low potassium levels and high blood sugar. Monitor potassium levels and blood glucose, especially in diabetic patients.
- Eosinophilic Conditions: Rarely, inhaled corticosteroids can lead to eosinophilic conditions or vasculitis. Be aware of symptoms like rash, worsening breathing issues, or heart problems.
- Hepatic/Biliary/Pancreatic: Corticosteroids may have enhanced effects in patients with liver disease.
- Hypersensitivity: Allergic reactions such as swelling, hives, or breathing difficulties can occur. Seek medical help if these symptoms appear.
- Immune: Use Zenhale cautiously in patients with latent tuberculosis. Those on immunosuppressants should be aware of the risk of infections.
- Monitoring and Laboratory Tests: Regularly check asthma control and potential side effects like bone density and eye health. Adjust the treatment if asthma symptoms worsen or do not improve.
- Ophthalmologic: Watch for cataracts or glaucoma, especially with long-term use. Report any vision changes to your doctor.
- Acute Asthma Episodes: Zenhale is not for the quick relief of asthma attacks. Always use a fast-acting beta2-agonist for sudden symptoms and seek medical help if asthma worsens.
- Asthma Exacerbations: If asthma symptoms persist or worsen, consult your doctor. Additional treatments may be needed if control is not achieved.
- Paradoxical Bronchospasm: Rarely, Zenhale may cause worsening of asthma symptoms. If this happens, stop using Zenhale and switch to another treatment.
- Pregnant Women: Use Zenhale during pregnancy only if the benefits outweigh the risks. Monitor infants born to mothers who use corticosteroids for potential adrenal problems.
Overdosage
Taking too much Zenhale can cause problems related to its ingredients. If you use too much, you might experience symptoms from the corticosteroid or formoterol.
Excessive corticosteroids can affect your body’s ability to manage stress.
An overdose of formoterol can lead to nausea, vomiting, headache, shaking, drowsiness, fast or irregular heartbeats, and other issues like high blood pressure and changes in blood sugar and potassium levels.
If an overdose occurs, supportive care is needed. In serious cases, hospital treatment may be necessary.
Cardioselective beta-blockers might be used carefully under a doctor’s supervision, as they can potentially worsen breathing problems. Monitoring adrenal function is also important in managing an overdose.
Drug Interactions With Other Medications
- Ketoconazole and Other Strong CYP3A4 Inhibitors: Taking these medications with Zenhale can increase the amount of mometasone in your blood and may cause more side effects. Avoid combining Zenhale with strong CYP3A4 inhibitors unless absolutely necessary.
- Acetylsalicylic Acid (Aspirin): Use caution if taking Zenhale with acetylsalicylic acid, especially if you have a blood-clotting issue called hypoprothrombinemia.
- Adrenergic Agents: Using other medications that mimic adrenaline (like certain asthma drugs) with Zenhale can increase the risk of side effects from formoterol.
- Xanthine Derivatives and Diuretics: Combining Zenhale with these drugs may increase the risk of low potassium levels. Be careful and monitor potassium levels closely.
- Monoamine Oxidase Inhibitors, Macrolides, Tricyclic Antidepressants, and Drugs That Prolong the QTc Interval: These drugs can enhance the effects of formoterol on the heart. Use Zenhale cautiously with these medications.
- Beta-Adrenergic Receptor Antagonists: These drugs, including some used for high blood pressure, can reduce the effectiveness of formoterol. Avoid using Zenhale with beta-blockers unless necessary.
- Anesthesia with Halogenated Hydrocarbons: There is a higher risk of heart problems when Zenhale is used with certain anesthetics. Inform your doctor if you will be having surgery with these types of anesthetics.
